I've seen guy who completed whole game without being harmed without using any cheats or tools (he used advantages of game mechanics that gave him temporary invulnerability like picking up item or throwing an enemy). But this run is clearly tool assisted, 6:12 it is impossible to change directions at this speed. But great gameplay anyway.

Now the rest mystery would be to find the level 3, 5 and 6 boss skips for The Punisher. Can't find information for this anywhere and I remember everyone does it at the arcade as a kid.
