    In a patient with attention-deficit/hyperactivity disorder (ADHD) and comorbid disorders, it is imperative to treat all disorders appropriately, and in terms of highest degree of impairment. This might mean that in one patient it is necessary to first stabilize the alcohol abuse, while in another patient the symptoms of ADHD might be more impairing than the underlying anxiety disorder. Additionally, some medications used to treat these disorders could exacerbate the comorbid ailment. Thus, care needs to be taken when choosing the appropriate treatment. An individualized treatment plan should be established for each patient based on his/her symptomatic portfolio.

    I have never heard before about dopamin depletation by chronic stress and a treatment with stimulants.

      I'd say it's cortisol not dopa/nri. i know for sure if not for stress and paranoia I'd be dead, in prison or homeless

    Regarding Chronic stress, why isn't stress reduction primary to this conversation or at least noted. Stimulants are not primary treatment for attention concentration problems caused by chronic stress, substance abuse, poor sleep or mood disorders. I can support the use of a stimulant in certain cases after other modalities of treatment are leveraged. A stimulant used too early may mask the actual underlying problem that may respond better the therapy or another medication. Placing a patient on a stimulant is a significant life and health changing event that should be reserved for patients that meet criteria for ADHD or as secondary or tertiary treatment of attention concertation problems or treatment resistant depression.
    Most recently (2020-2023) we saw the standing up of online prescribers making ADHD diagnosis for patients in under 30 minutes based on an ASRS and brief history that was focused specifically to include ADHD and that failed to consider other more likely diagnosis. Patients were being provided stimulants on a very weak diagnostic process which had negative impacts on the stimulant supply chain and inappropriately put patients on a stimulants. As prescribers we have the responsibility to provide treatment the patient needs not what the patient wants.

      follow the $$. My experience is that purchasing psychological services can be very difficult to get the proper provider. I am sure the field attracts a lot of frauds because you do not have to guarAntee the results. WithAHDH your clients are gullible parents who will do anything to get their children to "normal", these charletans who sell services are frauds IMHO, because there is 80% chance of turning the children to "normal" with medication, which per day cost is low, vs weekly "talk therapy with dubious results. I am a addict who has self treated my physical additions in 12 step programs that cost nothing, after having paid therapist with poor results; this is also the experience of the majority of addict I know. There are people who are taking money from people without knowing what they are doing but they promise these desparate people hope. Sleezy profession IMHO.
