Is the health declaration and custom declaration for on arrival passenger's or both...?

Arrival passengers have to fill both health and declaration form.

SSHP form requires seat no. What if we dont have it assigned while booking the ticket ? Can you help, please :)

Most airlines allow you to check in online 48 hours before, so you would get the seat number by then.
