    Mine shows Adderall, Xanax & Lortab are most therapeutic for me.

    so why does this person show Wellbutrin in yellow column with a "1" next to it? Wellbutrin's chief metabolite is CYP2B6 (which this patient's report says is normal). If the answer is because Wellbutrin uses CYP2D6 (which this person is a poor metabolizer) as secondary metabolite, then why is Fetzima and Viibryd in green column, as they also use CYP2D6 secondarily? Also, why is Trazodone marked with a "1" when its chief metabolizer is CYP3A4 (which this patient is normal with)?

      Most medications are broken down by more than one enzyme, and they don’t all play an equivalent role. The GeneSight test weighs the contribution of each pathway to categorize medications based on a person’s genetic makeup. In this specific case, the variation in CYP2D6 is expected to impact the metabolism of Wellbutrin, but not Fetzima or Viibryd. This could be because CYP2D6 plays a more minor role in breaking down Fetzima and Viibryd, or the other enzymes involved in breaking down these two medications compensate for that variation.     Why is Viibryd always in the green for everyone 🤔

      CYP3A4 plays a large role in breaking down Viibryd and the majority of people do not have a genetic variation in this gene.

      @@GeneSight That’s what I was thinking as well, one concern with pharmacogenomic tests is that companies that supply such testing may or may not have incentives given by pharmaceutical companies to more often or not suggest the use of one drug over another to patients. Another concern is the limitations of these tests in actually producing clinically significant improvement compared to TAU (Treatment As Usual). Because only a couple dozen of genes are sampled and there is much more that goes into an individual’s response to a certain psychotropic medication, we can’t really tell from this one test alone whether or not a patient might respond well to a drug or have more or less side effects associated with the use of a certain drug.

    So what happens if a person who has successfully been treated for a disorder takes this test, and the test tells them they don't have the disorder or the drug that has been working isn't given the "green light" by the test? What if the test tells them they don't have any disorder at all? What if a person has been taking a drug and it works, but the drug, like lithium which I assume is not tested for because it doesn't make drug companies a ton of money, is not on your list at all? Does this mean the diagnosis has to be changed or the treatment has to be changed?

      this test does not diagnose disorders. It assesses genes that can influence the effectiveness of mostly mental health medications. Green does not mean that the drug will work perfectly (as outlined in the video), it just means that the report did not find any possible risks based on the genes that the report analyzes. Also, Lithium is included in the report.

    I have high anxiety
    How does this GeneSight test help someone who has high anxiety and how are the medications listed on the test to help my anxiety with the right medication that has less side effects?

    Is polymorphism of the gene SLC6A4 rare?

      Based on the samples we have tested, 67% of patients have variation in this gene that may make them less likely to respond to SSRIs. This coupled with variation in one or more genes involved in breaking down SSRIs may be the reason you often don’t see SSRIs in the green category.

      The medication doesn't have genetic markers to be put into a category.

    Can a person be a rapid metabolizer and poor metabolizer at the same time?

      Not of the same gene but you can be an ultra rapid metabolizer for one gene and a poor metabolizer for another
