I'm continually impressed by how you're able to give us awesome informative content, give the viewers the basic idea of what a particular patient you're seeing is going through without giving us too much information about them. After digging into the internet I found that Canadian Healthcare abides by the Personal Information Protection and Electronic Documents Act (PIPEDA) which is similar to The Health Insurance Portability and Accountability Act (HIPPA) in the U.S. On another note: Congratulations on starting your 3rd year Medical Residency. I became a fan during the beginning of your 2nd year and immediately knew I needed to watch the backlog of videos of your 1st year and have enjoyed every minute of it.
I am loving how other people are participating in the videos! It is really helping get the big picture of how many people put together a larger puzzle all for the care of the patient.
Oh man, 3rd year! Congrats! I really love seeing how many different people at your hospital are actually directly involved in creating a treatment plan for patients. We can definitely learn something from you here in Germany.
I don't usually post comments. But, decided i would here. I was originally drawn to your videos because of believe it or not, your violin playing in the opener (which sounds good!)(my middle daughter plays)..... and since i used to be a paramedic, the medical cases you see and share with your viewers. I haven't been watching long but really like how you put the videos together and the medical content while protecting the patients privacy. I even laugh at some of the subtle edit things like the whoosh sound when you bring up the clock, etc.! I also challenge my diagnosing skills and try to figure out the patient problems from the presenting symptoms you describe, before you share the diagnosis and treatment plan...... please don't bill me for the training! Anyway, thank you for sharing the videos and putting in the extra work to edit and post these videos in addition to your tough work schedule.

I really enjoy your vlogs . Been watching since first year resident. I was recently in the London Ontario university hospital for 4 weeks with a bowel obstruction/twisted bowel & my Crohn’s disease active . I had a ressection of 19 cm taken out. I come from a small town. I was amazed at the “team” of doctors/surgeons who look after me. Everyone contributed & kept me well informed. Your vlogs helped me understand the process. With the older woman who was dizzy, can you tell us why your first instinct was to go to what medications she was on and not something like cardiac or diabetes? Thanks :)
Hi Robyn - great question! My differential was still very brought at that point (including cardiac, neurological etc.). I have an order of things I typically ask when I first meet a patient and medications is one of them... that's how it came up so quickly. But we did still order some testing to make sure it's nothing else underlying her symptoms. hope that makes sense!
If I had to guess, I would say she did that for a couple of reasons. (I'm an EMT-B not a doctor, so I could be off) 1. Older people tend to be on multiple meds just because they tend to have more medical issues, which you'd expect from a geriatric patient. So for older people, medication reactions aren't that uncommon. 2. Her symptoms could be the result of a number of things like you stated, but you'd want to start with the least invasive and cheapest possibilities first which would almost always be history, allergies, and medications. That would be my best guess for the approach to that patient.
Asking existing medications is pretty standard, especially if some drugs have the possibility of interacting with something that she's planning on giving

Just got diagnosed with Primary Pulmonary Hypertension, Pulmonary Artery Hypertension and Pulmonary Hypertension. Symptoms started one year ago. After one year of symptoms and seeing 5 specialists, getting diagnosed with SVT, Vocal Cord Dysfunction, POTS, MCAD and other stuff. You are so bright and smart and I hope you remember that some times, when you hear "hoof beats" it is a zebra 🦓. I'm 32 and am now in the fight of my life. It is a terrible disease.

I really enjoy your vlogs . Been watching since first year resident. I was recently in the London Ontario university hospital for 4 weeks with a bowel obstruction/twisted bowel & my Crohn’s disease active . I had a ressection of 19 cm taken out. I come from a small town. I was amazed at the “team” of doctors/surgeons who look after me. Everyone contributed & kept me well informed. Your vlogs helped me understand the process. Hi Siobhan I’ve been watching since you were in your 1st year. I never thought I could become a member of the health care team but I’m now a pharmacy technician in a community pharmacy and I’m studying for my state and national exam. So thank you for inspiring me. I know my job is no way near as important as yours but I love my job.
Without pharmacists tidying up 20 meds patients could get more sick than necessary.

With the older woman who was dizzy, can you tell us why your first instinct was to go to what medications she was on and not something like cardiac or diabetes? Thanks :)
Hi Robyn - great question! My differential was still very brought at that point (including cardiac, neurological etc.). I have an order of things I typically ask when I first meet a patient and medications is one of them... that's how it came up so quickly. But we did still order some testing to make sure it's nothing else underlying her symptoms. hope that makes sense!
If I had to guess, I would say she did that for a couple of reasons. (I'm an EMT-B not a doctor, so I could be off) 1. Older people tend to be on multiple meds just because they tend to have more medical issues, which you'd expect from a geriatric patient. So for older people, medication reactions aren't that uncommon. 2. Her symptoms could be the result of a number of things like you stated, but you'd want to start with the least invasive and cheapest possibilities first which would almost always be history, allergies, and medications.
That would be my best guess for the approach to that patient.

Asking existing medications is pretty standard, especially if some drugs have the possibility of interacting with something that she's planning on giving

Just got diagnosed with Primary Pulmonary Hypertension, Pulmonary Artery Hypertension and Pulmonary Hypertension. Symptoms started one year ago. After one year of symptoms and seeing 5 specialists, getting diagnosed with SVT, Vocal Cord Dysfunction, POTS, MCAD and other stuff.
You are so bright and smart and I hope you remember that some times, when you hear "hoof beats" it is a zebra 🦓.
I'm 32 and am now in the fight of my life. It is a terrible disease.
