Dear Dr. Renee: Thank you for your informative video. Can you please do a video about how to exactly use the estrogen cream (meaning, where do you put it? when? how often). There are so many different opinions, even among the gyns whom I have seen. Thank you.
I read through the comment section and I just want to say a big Thank You for taking the time and care to answer so many questions. You are quite remarkable! I found answers to all my own questions in the comments. God bless you.🙏🏾

How did you get on? I started my oestrogen cream tonight! I was worried about cancer but the advice I've listened to on here puts my mind at rest as it is not a systemic hormone. My doctor also made me aware of this but the side effects sheet is scary.

Thanks. It is very helpful. I underwent panhysterectomy at 31. Now I'm 51. Severe burning in the vagina. Was using estradiol, topical application. Discontinued due to fear of cancer. But now relieved to know that there's no problem.

Same here. I'm over 60 and can no longer take oral HRT. I can't sut comfortably. Some days I just want to shoit myself in the head. I can't wear panties, I'm just miserable and now Premarin has been discontinued and nothing else seems really good.
Thankyou for getting back to me. I used the pessories for a week & then twice a week as doctor said. The same with the cream but I’m going to take your advice & use them oftener. Thankyou for your advice, much appreciated. ❤️
Hi Dorothy. Just to clarify, this is not my advice to you individually, but a description of common usage. However, this should only be done after discussion with your own doctor who understands your history and clinical case. But it does give you a discussion point with your doctor.
Thank you so much gor this video! My question is , is there a need for progesterone in the vagina too, like the same combination already using as a cream? Im using estrogen, but occasionally I get the UTIs and some burning. Thanks again for taking the time to help us❤
I just found you. My doctor has just prescribed estrogen - estradiol cream for several reasons but she said it would help my rectocele. Do you agree with with that?
That was really interesting video thank you so much. I’ve not took HRT or anything since 2013, I had radical hysterectomy in Jan 2011 and suffered with HRT. They only offered me tablets ( headaches/bloating/fluid retention, constipation) now I’m osteoporotic and suffering bad brain fog, underactive thyroid too. I spoke to my GP he said try the gel you need to do something to protect my bones I’m 52. And frightened of what’s happening to my body as I’m living with Sjögrens & fibromyalgia too.
I’m sorry to hear that. I agree with your GP, gel will be absorbed directly into your blood stream, has very few risks (women on oestrogen only have a lower breast cancer risk) and it will help your bones and may also help your Sjorgens. I hope this helps. Good luck.

I’ve been on Estrogen 10 mg vaginal tablets for at least 2 years. I’m 65. The tablets have helped but it’s still painful during sex. Not as painful as it was before I started but still very uncomfortable with some pain, I use Replense Vaginal moisturizer also. I don’t know what else to do
How should I use local estrogen with a cannula or introducing it with my finger? In Spain they recommend promestriene. What type of estrogen do you prefer?
Hi . I’m a 72 year old woman with burning down below. My doctor has given me estrogen pessories & cream but after using this I’m not feeling a lot better. I use the cream & pessories twice a week. Can I use the cream every day? Please advise me. Thankyou
Hi. I am so sorry to hear this. So I always recommend starting with the cream and using it every night for 2 weeks and then 3 times a week thereafter. Then the same with the pessaries. Every night for 2 weeks and then up to 5 times a week. It takes time to rebuild the tissue. The cream can be used around the outside as well. Persevere and hopefully it will get better.
Sylvia Peace I am sorry to hear this. You need to go back to your doctor or demand a referral to a urogynaecologist because if you need vaginal oestrogen, it is irrelevant that you don’t want HRT.
Thank you so much. I have just started the cream about 10 days ago. Is it ok to use the cream more than once a day? I find the effect not lasting long enough some days.

Hi Dr Renee, What an informative presentation. I have two questions please, having read so much info on HRT, I am so interested and considering taking it, I am however 65years old ( missed out on HRT , I assume due to the 2002 WHI misinformation) You mentioned in the Blog "Vaginal Atrophy and topical oestrogen treatment" that you would not consider a 70 year old going on HRT, why would this be and also I suffered quite badly with endometriosis in my younger years and am a little concerned if topical oestrogen may trigger it off again as it wasin my bowel region. I shall continue working through all your blogs, great work. Thankyou
Hi. Thank you. NO i didn't say wouldn't but it would be a more cautious and careful decision. This is because a 70 year old has had 20 years -/+ and so could have/would have likely have developed heart disease and starting oestrogen can dislodge atheromatous plaques which is not good. It isn't a complete no no, just a careful assessment and discussion of risk. Any woman however, at any age can have vaginal oestrogen as this is hardly systemically absorbed. In terms of endometriosis, it is unlikely to trigger it at this late stage but the way of overcoming that risk is to ensure that you have adequate progesterone cover, which is essential any way if you still have a uterus.
Hi thanks it’s frightening my body has gone through so much with so many issues the severe tinnitus/vertigo and constipation fir me being the worst thing I’m living with. But now threat of bones because of the heart issues off the calcium causing problems it’s just one thing after another? I’ve just started using oestrogen cream in vagina because I have atrophy down there I had internal last wed omg 😱 the pain because I’ve got little vagina left. My hysterectomy has been life altering. 😢
So my doctor gave me a rx for Estrogen cream which would be great but my insurance won’t cover it and it’s $300 per tube . There is no way.. I been mad ever since. Like men can get Viagra just like that and it’s cheaper now more than ever but I have to suffer with pain? I’m 61 still young in a lot of ways . I don’t know about the ring or oral tablets. The doctor has not gone over any of those things when I told her I can’t afford the Premarin cream. Are they expensive too?
I’m sorry i am in the UK where things are very different. If you join the closed Facebook group run by Jane Lewis called vaginal atrophy there are ideas and suggestions from all around the world. Good luck.
Hi Thanks so much for providing the useful information in these vlogs. I wonder if you could let me know how long it takes for a vaginal cream to work? I’ve been on Ovestin once a day for a month and am now to start on twice a week but the burning after peeing has not yet disappeared. Many thanks
Hi Doctor, I have been using Ovestin vaginal cream for 6 months. It really helped a lot but when I had my health check up recently the ultrasound showed that my endometrium thickened ( 8 mm) , no bleeding however I decided to stop taking it . Unfortunately all my symptoms of dry vagina is back again))I am 50 and no periods for 2 years already. What do you think ? Can i carry on with Ovestin after two months break???
Hiya. The pharmacist is completely wrong. You don’t need progesterone at all in any circumstances for vagifem. If you later go to HrT, then the guidance in ablation is to do a 3 month progesterone challenge. If you don’t bleed when progesterone is stopped after 3 months then you don’t need it. If you bleed then you do. Hope that helps.
@@DrRenee Thank you very much Dr Renée for your answer. I am a healthy 61 year old and I will stop the Prometrium . I have a few hot flashes, but not excessive. Does progesterone help with that?
This is really informative but I have a question. I was diagnosed as having a vaginal contraction, I have had physio therapy which helped it initially, but it’s now getting worse, but I’m struggling to find any information about this online and my gyne is not bothered about it, even though this muscle contraction is getting bigger. I’ve looked in the menopause support group and VA group but there ifs only mention of physio. I assume I need to increase my local oestrogen? But that will take a while to kick in. Any suggestions or articles to read about this would be really helpful. When I was diagnosed I was told it was common! Not common enough to have any info on Google apparently!
I've been using this for about 10 years, works great, but recently got a yeast infection after 3 rounds of antibiotics (ear infection,UTI).Can you use this in conjunction with the yeast creams?
Can you cover pelvic floor probes like Tenscare and advise whether these improve blood flow and therefore improve vaginal symptoms. Thanks for all your posts.
After thirty estrogen decreases. What do you recommend if a woman wants to maintain and avoid that gradual decrease in youthful healthy vaginal tissue?
Exercise (especially anaerobic like short sprints). If you have a dog, run with it. They will make you do it 🐾 🤣 Weights, Royal Jelly. Kefir or live yoghurt with cultures that help keep good bacteria in your gut. A balanced lifestyle. Limited tea/coffee/alcohol. Blackout blinds on your bedroom windows.
Don't be nervous; there is almost no systemic absorption and to be honest a family history of thrombosis should not even stop you having oral HRT as through the skin there is no increased risk of blood clots. So definitely I would use the cream without any concern if I were you.
i only saw this video today but I have been on this journey for nearly 7 years, trying out all types of natural estrogens, since. I am currently using natural estrogen boosters and progesterone, prescribed by a Homeopath who is also a GP, which have helps a lot with the daily hot flushes and night sweats but the UTI symptoms come and go and are a menace. Since you made this video in 2019, are your views still the same?
I've been diagnosed with Vaginal Atrophy and Dr. prescribed estriol with tube applicator. But I cannot get the applicator inside my vagina more than about an inch because it's so painful. I try to push the applicator further inside my vagina but the pain is too severe. Could you give me any suggestions? Thank you so much!!!
Hi, I just felt I should offer this advice regarding the applicator: Toss the applicator, and use a finger to insert the cream. This way, there is less waste, pain, and aggravation.
hi.. I've been using te premarin cream for vaginal atrophy as a result of wearing a pessary for a prolapsed uterus... more often now I've been having a bit of nausea and strange headaches as a side effect wen I apply the cream.... I used it almost everday for two weeks and on sum days when I skip I start spot bleeding again.... is there a cream I can use to just keep the uti's at bay? like a disinfectant cream perhaps that will keep the linings from drying out and prevent uti s.. other than estrogen cream? sumthing I could perhaps use instead of/ or alongside the premarin cream... which will also reduce the side effects? Im struggling so much with this 😔
Can you please recommend a good Vaginal cream that is hormone free As I was put on Ovestin by my doctor but I have had to have surgery to remove a fibroid that Was 9 years ago. I have had another scan and Have been told I have another Smaller fibroid I am wondering if it Was the Ovestin that has caused the Fibroids. Any Advice would be much appreciated.
I am on hrt topical cream. Can I use the same estrogen cream vaginally? I am currently using a vaginal cream with dhea in it. Would estrogen vaginally be better.
Hi there, so I was still having my period in my 50's but when I was 52 I did go through breast cancer and reconstructive surgery to one of my breasts. I took certain supplements to make my menstrual cycle stop earlier than my natural body really wanted to... After that my period finally went away and I am now in full menopause. I have found that it's a bit harder to achieve an orgasm since my decline in estrogen and I am not a good candidate for hormone replacement because my cancer was fed by the estrogen. However, after viewing a few women on the subject it sounds like vaginal estrogen cream could still be an option for me since it does not seem to invade your system. I would really appreciate your professional opinion on my situation if you have a moment to spare.... Thank you in advance for any information you can share!!
Hi. I am glad you are through what must have been a real ordeal. So the first thing I always tell women in your situation is to read 'Oestrogen matters' by Prof avrum bluming. The re bvaginal oestrogen its certainly possible a systemic absorption is very low, equivalent to 1 HRT tablet each year of use. SO do discuss this with your oncologist. As another thought, it may be worth discussing testosterone with them too as if orgasm is the only issue, this may well help. Good luck.
HRT is oestrogen, which is the bit that solves the symptoms of menopause, plus progesterone for women that have a uterus to protect the lining of the uterus. This for many women solves the vaginal atrophy but if it doesn’t then vaginal oestrogen can be added safely.
Thank you do much for this video, Been suffering uti and vulva burning for 10 yrs after getting hysterectomy at 46. Ovaries removed as a result. On oral HRT and vagifem But still get vulva burning
Try Kefir or live yoghurt. They have cultures in them that help to balance bacteria in the gut. Anaerobic exercise such as short bursts of sprinting, helps increase estrogen levels. Weights support bones. Royal jelly is very good. You only need a small amount. It is a balancing act. Limiting coffee and tea helps me also.
I got Squamous cell carcinoma Vulvar Cancer a year ago had 36 sessions of radiation, 7 of chemo same time. Starts with a very bad itch, order, I then missed a small lump size of a inflated pimple so I ignored it a Mass was forming then started bleeding from the mass went to Primary doctor and he send me to gynecologist before the appointment I started bleeding really bad went to ER that's when I found out it was a big mass that had grown. He then told me to go to gynecologist on Monday I went and she did a biopsy it was Cancer going into stage 2. I am now almost fully recovered and just had my second surgery because Cancer came back after radiation and chemo although the cancer was gone back in Dec. 2023 CT pet scan confirmed it at that time it was gone. Good news I will have full recovery after this week's surgery, doctor did put me on Premarin 0.625 to put on Vulvar area and Libia for cell growth that show make me feel so much better. Squamous cell carcinoma Vulvar cancer is very rare and it's cause because of Menapause in older women I got it at age 65yrs. Ladies please don't get of you estergon even my Cardiologist told me why would your doctor tell you to get off of it if you were not having issues. Now my oncologist put me back on Premarin cream. Thought I would share my story
Thanks for the reply l have been having treatment for Interstitial Cystitis and bladder instillations helped as had inflammation which caused blood in urine, after cystoscopy showed pin point bleeding on the bladder wall. It’s quite complex and will read your blog for more information.
@@DrRenee I've just read that topical estrogen (Premarin) should only be used for a year as there are few studies of prolonged use. Also, I've read that the number one side effect of bio identical topical cream is weight gain.
I have urinary retention and have to catheter my self some time it burns or feel sore my urologist gives me pills for uti in of antibiotics but sore feeling don't go away please help me Thank you
DR. I have continuous itching even though using Elage oil ti wash, then Cicatridina Spray then the Elage cream. Is this normal? How long does it take to soothe the vaginal itching ?
You should't be using anything o your vagina bar water and if prescribed, oestrogen creams. There are lubricants such as Yes which are vagina friendly but I would not sue anything else.
Thankyou ,I'm such a worrier lol ,have had a sore vaginal opening on and off from a tear during labour 24 years ago. But I'm 58 now and feels worse, so will give ot a try x
I am in absolute misery. 54. My sex life is extremely important to my longterm relationship but I'm also experiencing frequent urinary tract infections and now burning. My doctor is prescribing me an estrogen cream to my local compounding pharmacy. I'm nervous about taking it but feel better after this video. I have to do something. So uncomfortable.
I have been using the estring for several years and recently switched to compounded vaginal estriol. Do I need to have regular pelvic ultrasounds to check the lining or uterus, etc... or is that only needed if there is bleeding (I have no bleeding)?
I asked my dr for topical estrogen as I have pain outside I haven't had sex in 8 years and probably never wl again but it came where I have to insert a tube is this what you mean by topical? Should I use it regularly? I am 46 and went thru menopause 42 to 45 I haven't had my period in a year and 5 months t
There are two types in the UK: vagifem and ovestin. Both are in tubes but ovestin is better because it is a cream that you place in a tube for insertion but can also rub around the outside, which you can't do with vagifem.
I’m utilizing prescribed topical/vaginal estrogen cream for a little over a mont and experience optical migraine, headaches/piercing headaches and nausea. When will these side effects taper off?
Thank you so much for your quick reply. Don’t know if I’m confused but should I choose not to take HRT, would I still need a form of progesterone to take when I use the vaginal oestrogen. Thank you once again.
I’ve been using oestrogen for over year and still experiencing pain and discomfort, I started off using the cream every day for 2 weeks and now use twice a week, do you think I need to use more frequently and if so how often. Also I insert the cream with an applicator would it help more to rub inside my vagina. I also use a prescribed vagina moisturiser but still am so uncomfortable. I’ve seen a gynaecologist twice the last time I saw her she prescribed a progesterone (can’t remember the name) but my Doctor wouldn’t prescribe it as it had been considered unsafe and been banned from use! The gynaecologist has now advised me to take amitriptyline for the pain which I have discovered causes even more dryness in the vagina …Jane Lewis mentioned this in one of her blogs. I much appreciate your help and reply. Thank you
I have been using a product like this but after about a week I started getting breast swelling and tenderness and bloated belly. What's going on and what can I do?
Royal jelly, you only need a small amount each day. Organic argan oil, or coconut oil as a lub. Eating live yoghurt with live cultures, or drinks with the same. Keeps the gut healthy. I am going to try pelvic floor exercises. I have a presentation for estriol but I can't see anyone saying it's great. Has it helped you at all or are you disappointed
@@annedavies2885 pretty disappointed. I'll try the Royal Jelly, I already make kefir from ow own cows milk and that does help. But I think I'm hormone resistant due to mercury toxicity
@@sixgunsue8 Good to hear that the Kefir is also helping you 👍 All the women I know on either the coil or some other form of HRT are having various problems. They are not a very good advertisement for it, bless them, and I am quite concerned about them. I have a book called The Bitter Pill (quite an old print) written by one of the Gynecologists working in the first contraceptive hormone trials with WHO. I am going to make a effort to read it, because I think there is a lot of information in there. Perhaps it will help me to understand if there are other ways to approach this natural process. Since I last spoke to you I have also read that anaerobic exercise, such as short busts of sprinting has been found to naturally increase estrogen in women. Sharing as much information as we can, freely to each other, is also a good thing.
Knew that years ago about sprinting. The WHO is a corrupt organisation that works for the UN. These are not benolevent institutions out to help mankind. Loom into UN Sustainability Development Goals and the lexicon they use. After 2020 I don't trust any of those gangsters 🤡 Coils were always a nightmare don't know why women use them. Bio identicals cream and homeopathy does work.

Thanks so much for providing the useful information in these vlogs.
I wonder if you could let me know how long it takes for a vaginal cream to work? I’ve been on Ovestin once a day for a month and am now to start on twice a week but the burning after peeing has not yet disappeared.
Many thanks
Sometimes you need to add vagifem too. They work on different receptors and so add to the effect together.
Thank you
Oestrogen affects other areas as well. Bigger risk for heart Dx, worse memory, worse skin, increased UTIs.
This video is just about the vagina, there are many on my channel addressing these other points, but for that you need oral/transdermal HRT.
Hi I ve had an endometrial ablation a few years back. I m starting Vagifem, my doc says I don't need progesterone, but the pharmacist said yes.
Hi. No you need systemic oestrogen for that I’m afraid. Good luck.
@@DrRenee Thank you very much
This is really informative but I have a question.
I've been using this for about 10 years, works great, but recently got a yeast infection after 3 rounds of antibiotics
(ear infection,UTI).Can you use this in conjunction with the yeast creams?
You can.
@@DrRenee Thanks so much for answering!
You need to taking high quality pro biotics. And eat live yogurt etc.
I was getting very bad uti I’m now using vagifem so good . Can you stay in vagifem for life x
It is perfectly safe for long term use. I’m glad it helps.
Thank you so much . I need to speak to my Gp next month to say I need to stay on this .
You can't stop the process entirely everyone ages and dies eventually.
I’m 66 and have recurring bladder problems.I was prescribed a cream.
I’m nervous of using it as l have a family history of thrombosis.
@@maryjones1my doctor gave me the exact same advice. Toss the applicator and use your clean finger
Im struggling so much with this 😔
Can I use the pill & the cream at the same time
Can you please recommend a good
Vaginal cream that is hormone free
As I was put on Ovestin by my doctor but I have had to have surgery to remove a fibroid that

I have had another scan and
Have been told I have another

Was the Ovestin that has caused the
Fibroids.
Good luck.
Thank you so much for taking time to give me some feedback and good guidance❣️ God Bless you 😊
What is HRT if you have all these symptoms..
On oral HRT and vagifem
But still get vulva burning
Try Kefir or live yoghurt. They have cultures in them that help to balance bacteria in the gut. Anaerobic exercise such as short bursts of sprinting, helps increase estrogen levels. Weights support bones. Royal jelly is very good. You only need a small amount. It is a balancing act. Limiting coffee and tea helps me also.
I am also going to learn pelvic floor exercises.
I need this im having incontinence and painful sex. Can I use this cream without telling my doctor?
wondering why l have burning pain and frequency in the bladder ?
This is a common symptom in vaginal atrophy as discussed in the vlog.
Thanks for the reply l have been having treatment for Interstitial Cystitis and bladder instillations helped as had inflammation which
caused blood in urine, after cystoscopy showed pin point bleeding on the bladder wall. It’s quite complex and will read your blog for more information.
Do you need to take progesterone if you are using a vaginal estrogen tablet only?
Dr. Renee, what happens after a year? How do we treat vaginal atrophy then?
586Rexford I’m not sure what the question means? Why would it be different after a year?
@@DrRenee I've just read that topical estrogen (Premarin) should only be used for a year as there are few studies of prolonged use. Also, I've read that the number one side effect of bio identical topical cream is weight gain.
Hi ,I have used it for 5 years ,no weight gain ,I am 80 😊
I have urinary retention and have to catheter my self some time it burns or feel sore my urologist gives me pills for uti in of antibiotics but sore feeling don't go away please help me Thank you
DR. I have continuous itching even though using Elage oil ti wash, then Cicatridina Spray then the Elage cream. Is this normal? How long does it take to soothe the vaginal itching ?
You should't be using anything o your vagina bar water and if prescribed, oestrogen creams. There are lubricants such as Yes which are vagina friendly but I would not sue anything else.
Im 4 days in to using the cream.....it feels like my ovaries are tender. Is that normal?
I have a uterus should I take bio identical progesterone on wrist at health store with my estrace cream prescribed by gyn
This is not necessary and not proven effective. If your estrace is for vaginal use for atrophy there is no need for progesterone
My friend used Ovestin and ended up with huge precancerous fibroids
Thanks for covering SLE
Hi will it make your weight change
Untreated menopause/ Yes. Treated vaginal atrophy? No
Thankyou ,I'm such a worrier lol ,have had a sore vaginal opening on and off from a tear during labour 24 years ago.
How did you go with the cream? Did it help? I feel your pain as I too suffer uti issues since I hit 42
With vaginal hrt can other over the counter lubricants be used in addition?
Definitely. Yes.
I have been using the estring for several years and recently switched to compounded vaginal estriol. Do I need to have regular pelvic ultrasounds to check the lining or uterus, etc... or is that only needed if there is bleeding (I have no bleeding)?
You don’t need endometrial protection if only using topical vaginal oestrogen.
Thx
How long does it take for these creams to work?
6-8 weeks
I asked my dr for topical estrogen as I have pain outside I haven't had sex in 8 years and probably never wl again but it came where I have to insert a tube is this what you mean by topical? Should I use it regularly? I am 46 and went thru menopause 42 to 45 I haven't had my period in a year and 5 months t
There are two types in the UK: vagifem and ovestin. Both are in tubes but ovestin is better because it is a cream that you place in a tube for insertion but can also rub around the outside, which you can't do with vagifem.
@@drrenee1300 thanks I am in the USA ca. Estradiol cream be put on the outside will it do any good? Or does it need to go inside
Janet Nixon Hi. No problem. No you don’t need progesterone with vaginal oestrogen
Dr Renee That’s great, thank you.
Is prednisone aHRT it’s not helping me
No. Prednisone is a steroid so won’t help oestrogen deficiency.
Thank you
Also, what about the "Mona Lisa" treatment?
586Rexford the study results on Mona Lisa are very encouraging indeed.
What is your view on fagifem
This whole vlog is literally my view on vagifem and estriol cream..
Are side effects minimal also with tablet inserted vaginally - asking for my 88 yr old mom....who has lots of health issues...
Michael Patrizio totally safe at any age and lifelong with no real side effects.
@@DrRenee thank you.....I did notice her blood sugar went up ie fasting this morning was 194 - so after 5 days on the estrogen tablets - wondering if it impacts blood sugar in diabetics?

I use Pharmaceutical Estradiol vaginal cream BUT 3 of the ingredients are questionable and I am concerned if I Continue to take this product twice a week for many, many years to come.
.
My husband's pee burns for days after sex) the day after the night I use the Estradiol cream.
.
I would switch to compounding but but it is not regulated and chnages in suppliers and ingredients change each month along with the person filling the script and no oversight, there is no gaurantee what you are getting.
.
Is there a pharmaceutical brand that does not have the ingredients below?
.
-sodium lauryl sulfate: (irritant, toxic to aquatic organisms)
-methylparaben: (low to moderate health hazard)
-tertiary-butylhydroquinone: (harmful, banned in some countries) Tert-butylhydroquinone (tBHQ) has been commonly used as a synthetic food antioxidant to prevent oils and fats from oxidative deterioration and rancidity due to its potent anti-lipid peroxidation activity. According to the Centers for Science in the Public Interest (CSPI), a well-designed government study found that this additive increased the incidence of tumors in rats. And according to the National Library of Medicine (NLM), cases of vision disturbances have been reported when humans consume TBHQ. This organization also cites studies that have found TBHQ to cause liver enlargement, neurotoxic effects, convulsions, and paralysis in laboratory animals. The additive TBHQ is banned in Japan and other countries - yet it's found in hundreds of American products in our grocery stores.
