  • After more than 21 years, I still get multiple questions a week about hormone replacement risks, specifically regarding breast cancer, but also fears of other disease risks. Today my partner Dr. Leah Antoniewicz ‪@drleahatoz‬ and I are revising the risks and benefits of hormone replacement, and the curious history of the WHI (Women's Health Initiative) study.
    We'll be discussing the science behind this very emotional topic, and I hope to lay it all out there for you so that you will have the best information to present to your own doctor and make your own informed choices!

      Certainly, it is something to consider for most people, although not for everyone- for the majority of women the risks are fewer than the benefits!

    I was on anatrozole. I kept having UTI's. It was my cancer doctor who prescribed vaginal estrogen cream that stopped my UTI's. He also took off the hormone blocker.

    Dr Hardwick Iam on the estradiol patch and progesterone pill vaginaly for two years now. I feel great! Because of polyps in the cervix, my doctor recommended a progestin IUD? Is this a good option? Are there any risks or side effects? She said it will deliver progesterone topically where needed and prevent new polys from appearing?Thank you so much for all the knowledge you so generously share with us

    How do I find a doctor to prescribe me with HRT. I had stage 1 and all removed and good. But when I try to get back on hrt doctors tell me they cannot or they will be sued for malpractice!! Help

    If you are overweight age 52 can you take HRT? Slightly elevated cholesterol but no other health problems and non smoker.

    I’m 65 and only sleep 4 hours for the past 15 years. After 3 months on low dose estradiol patch and 100 mg progesterone tablet my breasts were severely painful. I stopped the hrt bc the obgyn didn’t want me taking it after 15 yrs post menopause. I now have osteoporosis and feel lost at what to do. Can you suggest?

      I'm 82 and use birthday and have for decades. My osteopenia had stayed stable since diagnosed at 45. My new GP was horrified I 'm still on hrt so I stopped and very rapidly descended into osteoporosis. I've now started hrt again . I wonder if the body identical method would cause you fewer side effects ? I sleep better when I'm on it.

      As we mentioned, there's no reason to stop HRT because you've been on it for a certain period of time and the dose can be adjusted to address breast tenderness

    Is there a minimum amount needed to prevent heart disease and osteoporosis?

      The answer to this question has never been established, but we use enough to resolve symptoms and generally for most patients that is an estradiol level somewhere between 40 to 80 but there's a lot of variation between individuals so it's great to have a doctor manage this closely

    Seems like I see a lot of women in their late 30’s early 40’s getting breast cancer. Wouldn’t they still be producing enough estrogen to prevent getting breast cancer?

      There are so many factors that lead to the development of breast cancer and often and younger women it's genetic- so certainly many things to consider. There's not one factor that causes breast cancer. It's a combination of multiple things- much of them we don't understand!

    Have you seen women with asthma have worsened symptoms typically on estrogen?

      I have not seen that in a patient nor have I seen that discussed in the literature personally. That would be hard to understand since it is present in large amount all of our lives between age 12 and 50 for most patients.

    You said, don’t take estrogen by mouth or it’s not suggested to what about a troche?

      A troche is designed to dissolve in your mouth and not get into your stomach so that is another option. However, it's not my preference because we don't see very stable levels with troches. Is very variable absorption.
