How Botulinum toxin injections can worsen neck instability and pain - a Prolotherapist's perspective

  • Ross Hauser, MD explains the answer to a common question we receive about botulinum toxin injections for headaches and whether or not we do them in our offices or recommend them to our patients. As he explains, botulinum toxin injections can worsen neck instability and pain which has the potential to worsen headaches, occipital neuralgia, and other symptoms associated with cervical spine instability. Because we are a Prolotherapy practice and try to help patients do what they can to strengthen and repair tissue, these are not something we use in our offices. If you have had good results with these injections, and your symptoms resolved and allowed you to get back to your full function, that's great. Please understand that we see many cases in our office that did not. Dr. Hauser is explaining why that happens.

    Thank you for confirming that my Botox injection made my pain worse. My ENT Physician made me feel like it couldn't happen. Yes it did.

    Thank you for covering this topic. I have a long term severe neck injury that has sadly been covered up for 21 years. I get migraines from this injury. I had one Botox treatment for migraines and am now suffering from choking at times. No more Botox for me. Thank You!

    Dr. Hauser, you just saved me from going down this path… I’ve had moderate to severe pain in this area and already have some nerve canal narrowing in the C2-C3 region, I can’t imagine how badly I would have messed myself up even more.😬 Thank you for this video and this is horrifying example.🤯

    I have severe TMJ (jaw surgery) and neck pain (2 whiplashes) and botox was the solution they offered me after 2 years of physical therapy that bear little fruits... But I'm so scared of botox, this video comes as the right time! Thank you for informing us 🙏🏻🙏🏻🙏🏻🙏🏻 I'm trying acupuncture, it works well but you need to do it frequently at the beginning while keeping physical therapy and osteopathy checkups. Hope everyone gets better ❤❤❤❤

    I just got Botox treatments two weeks ago for chronic migraines and cervical dystonia. But honestly been feeling worse and I didn’t know why. The pain has increased so much and I feel miserable & the migraines are back in full force. I thought I was crazy that the pain is just worse not better.
    Just recently diagnosed with cervical dystonia but I originally went for Botox for chronic migraines.
    I live in Maine in here on the East Coast these doctors haven’t heard of much cervical dystonia it took me two years to get a diagnosis.
    I finally found a neurologist that diagnosed me and his course of action was to treat me with a Botox. He said after two weeks I would start feeling really good.
    I am in such severe pain and it’s hard to function most days. I’m hoping I can get a consultation at your office at some point. I’m glad to know there’s other alternatives besides Botox because so far nothing has worked for my chronic pain. I live in Maine now but we are moving to Florida in the fall probably by September or October.

      I’m so sorry. I get this too! I have bad forehead wrinkles and am very expressive. I’m thinking I won’t get Botox for awhile. Maybe never again.

      Hi Nicole! So this post is from 11 months ago, how are you feeling now? Your post scared me because I just got the botox injections yesterday in San Francisco at my neurologist office for chronic migraines. He said it takes about a week or so to kick in. I already suffer from chronic back and neck pain as it is (arthritis) and can’t afford to feel worse with this botox. I alternate between Sumatriptan and Maxalt pills (insurance only covers 9 pills a month of each and I get 20+ headaches a month), they work at the first onset of the headache, but if this botox helps to prevent the migraine pain I won’t have to take all these pills all the time. I also take Percocet for my back pain for 11 years. Good luck and let me know if you’re still doing the botox since it’s every 3 months and if your feeling better.

    It is exactly what happened to me, I had the botox treatment on my neck and trapezius to remove muscle tension, after 3 days everything got worse with terrible pain from my neck through shoulder blades, shoulder, arm and fingers, my nerves were inflamed So much so that the nerve was trapped between my vertebrae, I stopped lifting both arms, it's been a month and my chiropractor has helped me a lot but the pain and tension are still there

      Hi Robert, Hope you're doing well. Thanks for reaching out. Prolotherapy injections are not done into the muscles, but at their attachments to the bones- it targets the ligaments and tendons that stabilize the joints. Usually, these patients need all of the posterior tendon and ligaments attachments and facet joints injected to try and stimulate a more comprehensive healing response.

    Left shoulder injury from a slip and fall reverse zombie.. made my neck issue the Botox had lasted about 8 years. I got them from the dentist.

    What are your thoughts about using botox in the shoulders for overdeveloped trapezius muscles?

      Hi Felorina, Thanks for the question. We don't like it if it has to do with the trapezius protecting the spinal cord and other structures in the neck due to instability of the ligaments. Drs.     I have extreme dizziness. Head pressure and neck stiffness That last every moment that I'm awake since I got botox. If you have any recommendations on how I can help resolve this issue, I would be forever grateful.

    Can prolotherapy help even if I think my case is a reaction to benzodiazepine withdrawal in combination with an asymmetric jaw (i.e. short right jaw, clenching in/out of sleep resulting in excess tension via right SCM pulling my head to the right). When my condition was worse I would often feel a muscle spasming on the rear left side of my neck just behind my skull that would practically force my head/neck back into it its crooked rightward facing.

    Gosh I beg someone to reply. My mother receives Botox injections into her vocal cords for spasmodic dysphonia. She has since developed extremely debilitating occipital neuralgia and neck pain. The doctors attribute to the pain to severe degenerative disc disease. It occurred to me the two may have a correlation but I googled a bit and asked her doctor and I was basically reassured that there would be no correlation. But….. does anyone know if they could be related because my poor mother is approaching suicidal thoughts due to the pain and terrible prognosis she has been given. I just want to give her hope. I am 48 and I don’t care if I am 98, I need my mother.

    Hi I have a question. I’ve had a little bit of Botox in my sternocleidomastoid muscles for neck pain but it is now understood that the muscles is actually weak, causing the problems in the neck. So now I have to strengthen it. Will Botox not allow strengthening and disable the muscle? Thanks you 🙏🏼

    A doctor said I clench my jaw because my jaw looked stronger on one side slighly and did botox to one side, what followed next 3 years was my whole body collapsed and I was unable to work, I had stomach issues , sibo , burping, constipation, hip issues, tmj and headaches ( never had a headache all my life til 29 years old...EVER. I realized my muscles were adapted and botox misbalanced things and crushed my vagus, since then its been hell trying to balance things on top of at the same time quitting SSRI, both botox and ssri are pretty unpredictable to these nerves...It took me 3 years to research everything and doctors dont understand how complex these things are, let alone assume stomach issues can be from the neck and jaw...They think the gut consists just of the stomach and not tied to brain and the vagus so they try to treat it with acid blockers and antibiotics which make you sicker. Wish more people just looked in to their body, holistically without jumping blindly into dangerous things like antibiotics or things back surgeries that dont work

      The actual botox that's injected is out of your body within days. The byproduct it leaves behind is gone within months. And it's effects are _very_ localized (within a few centimeters at the MOST). It's an extremely safe medication that's very well understood and tested.
      Not to mention, the systems and anatomy you're talking about aren't even related. It would be like saying that you're experiencing stomach ulcers because you broke your ankle 2 years ago.
      I'm really sorry to hear you suffered. I've been there myself (going doctor to doctor for a diagnosis). However, just because you had issues that started around the same time, doesn't mean they're related. So I would consider it just a bad coincidence because it definitely wasn't caused by the botox. Good luck with everything regardless.

      This sounds like toxin spread from iatrogenic botulism poisoning, this happened to me from Botox for TMJ. There are warnings about toxin spread in the black box warnings for the product

      ​@@Anne_Onymous Botox wears of muscle weakness from botox doesnt leave in 2-3 days. I am aware its safe for the body nevertheless young people doing this looking ages older and lots of people developing tmj after botox cant be ignored. Its still a risky unnecessary procedure. I hate when medical professionals completely downplay the effects. Im a scientist, I know that risks are low but they DO exist. If its 1% perhaps I was in that 1 percent and it was a wrong decision and likely was registered badly. I heard people dying from chiro adjustments, doesnt mean chiropracters are unsafe, there are very very small percentage of risk anytime you manipulate your muscles / nerves. Especially your head region. The neck and jaw area is a sensitive area. I had already clenching and the wrong injection threw of the balance of my facial and neck muscles if that makes sense. We analyzed with a neurologist. With the right exercises and understanding, today Im super healthy. But I have zero doubt my headaches were from botox.

    Will the symptoms revolve as the Botox wears off over three months? I am a month into this and absolutely miserable. My doctor said he had trouble “funding correlation” between the injections in my neck and sudden debilitating neck instability the next week. I’m miserable. Thanks

Unfortunately, we do see this after Botox, and because cervical instability is a progressive disorder, it is not something that we've seen resolve on its own.       Hi Dr. L, Prolotherapy is used instead of Botox for people with TMJ pain and degeneration. It is not given into a muscle though. Prolotherapy is given at the fibro-osseous junctions in the body- where the ligaments and tendons attach to the bone, as well as intraarticularly.     I'm feeling pretty helpless I was diagnosed with ndph and I have been in chronic pain for over three months now with no resolution at all whatsoever I'm desperate and was planning on trying Botox to try and see if I can get rid of this pain but now I'm really worried if I should do it or not I've tried everything else and nothing has worked

      I suggest a few months of physical therapy. It’s the only thing that helped me get from feeling like I’m dying with very little quality of life to being able to be productive with variable day-to-day pain. Mental health therapy can, of course, also be empowering or cathartic. Hang in there!

      You can’t paralyze nerves with Botox, but a nerve block might help. A doctor will inject numbing agents into either the Lesser or Greater occipital nerve or both. There are also some medications, like Cymbalta, that can help with nerve pain, or other medications that affect the GABA pathway (if I remember correctly).

    Hi i had dyport in my brows and glabella april 18th and ive been having nerve pain all over my body and fatigue with blurry vision. It feels like my nervous system is on fire. Will this go away? The injector said they don't t know

      So, it has been 4 months since I had the botox injections. Thank God my migraines didn't get worse. My neck feels ok.
      I had migraines since the procedure but just 1 very bad. Usually the happen during my PMS and they last 2 or 3 days. But the ones I had after the botox are not as bad as it was before.

      I just had the 31 injections yesterday here in San Francisco by my neurologist for daily headaches, hopefully there will be some improvement in the migraines. He said it takes about a week to start working. It’s been 24 hours for me and feeling kinda flu-like today, did you feel this also within your first day of the injections?

    Thank you for this information! I have Chronic Migraine, EDS, POTS, and suspected CCI. I was also in a bad car accident 8 years ago. I am about to get Botox for Migraine for the first time but was hoping the doctor would allow me to skip the neck and shoulder areas since I already have neck issues (mild degenerative changes and mild hypertrophy and stenosis). I've had Botox before from a medspa in my forehead and it has helped with the migraine pain. Do you think it would be safe to do Botox for Migraine in all areas except for the neck and shoulders? I will be looking further into Prolotherapy. I hope one day to visit your clinic.

      Hi Nicole, we cannot recommend any treatment without first seeing a patient as it would not be appropriate.     I got it for Cervical dystonia, I wanted to give me for my migraines too, but the Sternocleidomastoid muscle on my left side is gone and now I have more pain then I had and my head still twists . I was given Procedure record: 6 mL of preservative free 0.9% saline is used to dilute 300 units of Botox. Got the shots in the back of my neck as well and to hold my head up now hurts so bad I have to brace it

Our video is referring to botulinum toxin into the neck muscles as we've seen more patients coming in for unresolved neck/migraine/pain and stiffness issues that is worse after these injections into the neck musculature.       I got dysport for my 11's, crows feet, and forehead lines. The very next day I felt tenderness at the base of my skull near the occipital that progressed over 2 weeks into debilitating pain radiating down my neck and traps, accompanied with dizziness. I've never had neck pain in my life prior to this.

    I am in desperate need of help and the Canadian health system over here is just not helping me, I'm afraid I will die if something's not done soon. After an infection my neck started to stiffen up and won't stop without attivan which I take 4 times a day and the stiffness worsens when I eat and then it's really hard to breath. I don't know what to do, I've seen doctors here but they have no clue. I am in such pain on the right side under my ribs and they did ultrasounds and found sludge but won't remove it... I'm in really bad health and I'm scared. I'm desperate.

    I had botox in my neck for chronic neck pain last Tuesday, so a week today. I am in extreme pain now and the muscles are so weak I can barely move😢 please please tell me this will wear off soon

      Hi @trishaszucs7065 we are so sorry to hear that you had a negative experience. Although botox injections can help people, botulin toxin can create weakness in the muscles and cause worsening cervical instability. Worsening cervical instability is why many people have headaches and symptoms that worsen. The botulin toxin has atrophied the muscles.

    When the pain seems to manifest around the eyes/temples/jaw and seems related to the lower back of the skull (where it might be bad to get botox injected - would it not be helpful to alleviate some of the pain, by getting botox in the face/temples/jaw? If just it made the pain manageable, i guess one could keep treating the neck-issues with exercises and manuel treatment?:)

      This is a great question, it really depends on the situation and severity. We have had several patients who receive botox injections and it relieves their pain, headaches, etc. but we have also seen the other side of it; making the symptoms worsen.

But after reading the article, where it is obvious there are some potentiel problems related to getting botox injected in the neck, i still figure it would be "safer" to just get in the temples/forehead/jaw, without ruining the possibility to work with the neck at treat the root-cause at the same time? I reckon that some of the other sideeffects are still a possibility, if someone in generel respond bad to the botox. But do you never give just "some" botox (not in the neck) and then continue the primary treatment to address the root cause?:)

      Exactly. I was diagnosed with dystonia years ago. I've tried several oral medications. Nothing else helps at all.

    Does anybody have problems not jydt with neck stiffness but also pain in the middle of their back where that it feels stiff and you cant hardly move or bend over this started happening to me sfter i got my first botox shots i got 32 shots my neck my head and my shoulders

    That’s what I was thinking. If it paralyzes a muscle from contracting in the face, it would make since on why it would cause problems in muscles around your neck cause instability in that area. So not sure why doctors are still injecting in the back of the neck like that know what it does.

    I have had Botox treatments at the base of my skull for 4 years. A new dr. did it this time and it felt deeper. After a couple weeks everything went crazy. Numb tongue, dizziness, swallowing issues. Shakiness. I am a mess! I have 3 plates and 10 screws and already had a hard time holing neck up.
    Can you get better after the Botox wears off? I am worried

    What is your opinion on botox for pelvic floor dysfunction/vaginismus?

      Non medical opinion here, but I’m pretty sure pelvic floor dysfunction is a result of weakened muscles, so atrophying them with Botox would not help. What you need instead are strengthening exercises (perhaps with kegel balls, but definitely talk to a physical therapist or OBGYN). Vaginismus is more in my wheelhouse as a mental health counselor in training. Vaginismus usually a psychological resistance that causes muscle contraction (there’s good representation for it in the Netflix show Sex Education, which I highly recommend!). I recommend working through some of your conscious or subconscious cognitions with a sex therapist and NOT pursing Botox as that would weaken your muscles to get rid of the vaginismus, but it wouldn’t help any psychological causes at the root of the problem and could perhaps cause more mental harm than good. Additionally, muscle contraction is one factor that leads to orgasm, so I’m guessing you’d like to not weaken those muscles.

    Hello Doctor. Is it possible that c2 rotary can cause straight neck? Is it fully cureable by chiropractor's adjustment and these symptons such as tinnitus can go away?( Cox Technic )

      Yes it is possible I have exact problem and I finally found a good chiropractor that is fixing it . you need a very good chiropractor to fix it not just any chiropractor . I go to one that specializes in Blair teqnique. It's working !

      Hi Michael Rex, Thanks for the question. We see straightened curves or even S-curves frequently. In our offices, we treat them with a combined approach of Prolotherapy to strengthen the ligaments, as well as curve correction. This can include some chiropractic work as well as individualized head/chest weight protocols that help encourage the normal c-curve. In our experience, many patients with more advanced cervical issues do well with a combined approach to address the skeleton, muscles, ligaments, tendons, and nerves and nervous system.

      ​@@Antonioskymusic First symptoms started 10 years back it was head tremor while playing snooker. Then whenever I was in stressful situation like giving exams I had head tremors. I thought this is due to my neck spondylitis. 5 years back while working out i got injured and i got neck spasm which never went till now, i also did neck streching as soon as I got injured in gym from a physiotherapist which should not be done in my case bcoz it increases my spasm and dystonia. I believe i have cervical dystonia due neck injury 12 years back. One year back i started having head tremors continuously bcoz I was taking wrong anxiety medicine given by gastroenterologist. So i went to psychiatrist and he gave me medicines which i took everyday and my head tremor had almost stopped. My psychiatrist told me to do meditation, pranayam yoga and shavasan yoga daily. That iceman(i forgot his name) breathing method also helped me from utube. Muscle relaxation method video from utube also helps.

    If you want real results with all due respect to Dr H, strengthen the cervical muscles, that’s the best way to stabilize the neck and Stabilize the atlas, tightening ligaments won’t help if your muscles are weak.

      I had 200 units of Botox placed in my posterior neck, traps.,and lumbar spine 2 weeks ago and now I have bobble head with weakness spreading to my legs and arms.
      Are these normal symptoms?
      Would a parasympathemometic medication be appropriate if I slightly overdosed?
      My neurologist failed big time with discharge instructions.
      Thanks for any advice.

      I called the FDA and was told about the side effects of Botox and that I most likely was OD.
      I’m taking all adl in stride and very slowly.
