That eel looking thing is a leech. They flatten their bodies when they swim. The woolly bugger is designed to mimic them. Many fish species find them irresistible.

Austin Simpson Buggers are a reasonable approximation of a leech, fry, or small baitfish- rolled into a single fly pattern.. If you think about it, many classic fly patterns work because they vaguely imitate a number of critters by not being an exact representation of any one in particular.. The Adams, Prince nymph, Copper John, Walt’s worm all come to mind
