      Do u consult online? I am suffering from back pain for the last 10 years and now for the last 6 months the pain is in my left hip and calf muscle. If I walk 100 m it starts paining and I have to sit somewhere to continue walking again.

    For me this actually decreased the pain almost immediately. I’ve been dealing with pain when bending forward for months now. Today I was finally able to to try and touch my toes with no pain. Thank you !

    After spending a couple years dealing with piriformis pain & being told to stretch, yet not getting any long term relief, I came across another UA-cam video that helped me tremendously. Not everyone with “piriformis pain” has a tight piriformis and stretching is detrimental. Some of us have a weak piriformis & need to strengthen it. Since beginning a few strengthening exercises, my pain is all but gone. If stretching doesn’t seem to help, considering searching the video on “stop stretching.”

    I have no idea why your video showed up in my feed. I have not searched for Piriformis issues or Sciatica. But man o man I am happy it showed up. Not only did you get to the point, explain it very well but when I did the movements you showed. I realized that you proved what I suspected was the problem. My Chiropractor showed me my latest x-ray and what he said was a possible problem, you said would cause my pain issues. Thank you for putting out this video. It gives me hope that I can get rid of some of the pain I am in all the time. It is not screaming pain but that constant throbbing with shots of intense pain when in certain positions. The road to mending may have just gotten easier. Thank you again.

    I am 6 months pregnant and after running the other day, I felt this pain deep in my glute radiating to the top of my back, to my knee, and to my toes. Thanks for this video!! I have been over-stretching and incorrectly thinking the pain (good stretch) was helping. I just found your channel and it’s great very informative. I would love to see some content for during and post pregnancy.

    Great information! I get that pain when my hamstring gets to tight from neglecting it and from to much cycling. It took me to long to figure it out but a world of difference when I found the main issue.

    This works. I have a herniated disk, and rehab has been a regular routine in my life. Recently with pandemic, I found myself sitting at home for a long period of time. Got a bit complacent about the core workout. and boom I got pinching nerve pain again, travelling down my right leg.
    I remember all the other core workouts, yet I forgot the most basic extension exercise. I now remember the basics that my physio taught me a few years ago. My pain is magically gone after a few extensions.
    Herniated disc symptoms get worse when you gain more weight (fat, not muscle weight). Good luck to those who have nerve pain. This extension plus walking for 30min~45min a day should strengthen your lower back. Walking "activates" the small muscles around your disc L4 and L5. You can feel it if you place your fingers there as you are walking.

    Oh thank you! At 3:52 I did that stretch and it felt so good! I had a total hip replacement since 2013. 7 years now and I'm 45. I dislocated my right hip in 1988 at age 13 so you can image my pain of arthritis since age 15. Doing this and standing up now I know my pain in tight hamstrings. Plus shoulder, neck and upper back, Sadly, ever since this dislocated right hip, it affects my left knee and now I have old arthur in my left knee.

    Your video and explanation is excellent!
    I had a posterior bilateral L5 Laminectomy surgery 3 years ago and a year and a half ago had a synovial cyst removed off of my right hip, this was the point of entry that my neurosurgeon used.
    After taking multiple painkillers to my main Dr. I went back to the neurosurgeon and am now taking steroids which are immensely helpful.
    Even with my back issues, nothing will keep me down! I have a tear in my quad and after I felt the care from my synovial cyst removal on my right upper buttocks, it felt like the muscle in my leg dropped, downward!
    I don't believe I have any nerve pain, and my MRI shows that there is no reoccurrence of any synovial cysts!
    I think your video is excellent and I think I am going to need time to heal. We show it in your area I would come to you for physical therapy. Great job!

    I am glad I came across this channel! I have been to so many doctors, chiropractors, and they all tell me it is sciatica and lumbar disc issues but (the worst part) they tell me to do the exercises that you say not to do! No wonder I am in so much pain at night!

    Doctor you have saved my life. I tried you simple thing by laying face down as in the video. It takes away My lower back pain in a week. Years of regular back stretching only bring Temporary relief and has made my lower back worsen. I’m so grateful that your posted this video And I found you.

    Music to my ears! I've got a chronic L5 pars defect, with reduced disc height. I've had intense muscle pain/spasms in what i now the Piriformis. I've felt like i wasn't able to run for the rest of my life. Im finally starting to get a handle on it and ran my 5km goal! but id love to know more about long term management and treatment to stop the pain and issues that sneak back up over time.     Well explained! Ive wondered about several points you've made about the spine rounding. I cant seem to get satisfaction from chiropractors and physical therapists where I live. I did use the best chiropractor Ive ever seen in Orange County but he retired. Wish I lived close by as I have suffered from piriformis syndrome for 20 years and now I have a problem with the lower sacrum area now.

    Thanks for this video - if my Piriformis is aggravated by outdoor running, should I follow a similar course of treatment; improving posture, etc.? I would like to continue to run, though I seem out of commission for 3-4 days after each run. The pain is particularly noticeable going down stairs and high impact/speed activities like running. Are there other strengthening/stretching regimens i should be doing? I should note that I have flat feet/pronation as well. Thanks in advance!

    This is fascinating. I've been dealing with piriformis tightness for years and even went to a physical therapist, who recommended stretching the piriformis and psoas. The tightness continued on a consistent basis, along with low back tightness, and now I have full blown sciatica. I am in the process of being worked up by the spine people. I wish someone had recommended extension exercises a long time ago. It might have prevented my problem.

    You've mentioned strengthening the lower back muscles, since they drive the piriformis' muscle pain. What about the disc herniation and the little hole getting smaller, which cause the lower back muscles pain? Do they go away with the back strengthening exercises? How do the herniation and narrow hole get treated?

      That's a lot of questions. Disc herniations and hole closure are separate topic and different drivers of nerve pressure to the piriformis muscle. I'm a trucker who hasn't been using Cruise control while driving long hours, and as a result, my right Calf muscle has lost so much strength that I cannot now do calf raises on my right foot.     This is absolutely helpful! I have Piriformis symptoms that came on with a vengeance about 2 weeks ago. Though I’ve made progress, I still have pain. I know I have scoliosis and the stretch on the belly pushing up does help; however a walk more than 4 minutes or so gets pretty intense with pain. Can you comment or direct me to exercises/stretches that would help with the recovery?

    Loved your simple and direct explanations on the Piriform Syndrome. After an injury my hips movements became very tight, before I was able to seat for 🧘‍♀️ on easy pose, and seat on the floor for hours now is very uncomfortable, painful in the right side by the Piriform muscles, also when walk for over 35 mins start felling pain.

    This was the best explanation I've heard from any professional and I've been to a lot over the last year. I was diagnosed via MRI with L4-L5 and L5-S1 Disc bulge which is causing my piriformis to be tight and give me sciatica down my leg. The problem is, arching/extending my back actually makes it feel worse which seems to be counter intuitive to everything I've read and been told. Should I then be rounding my back instead?

    Out of probably close to 50 videos I’ve watched you diagnosed my problem perfectly! So I know the real problem. However, I have no idea how to fix it. You didn’t give any exercise to strengthen my body to fix the problem.

    I drove 11 hours in Australia with two breaks. That’s what did it. Back is already problematic due to compression work injury.
    L5,L4,L3 are all compressed vertebrae and shaped like wedges.
    Sternum fractured, 3 ribs fractured, pelvis seperated from the sacrum on the left side completely and was still half attached on the right side of the sacrum.
    Pain has been my middle name since falling 8.5 meters off the ladder (linesman/Electrician in Australia) in 2-2012.
    At one stage in my return to work program I could not even sit on these uncomfortable “ergonomic chairs”. They were like hell to sit in. I had major issues going to the toilet, the pain brought on anxiety, profuse sweating.
    Thankfully that is behind me now. Just driving, even short distances causes immense pain.
    I have been able to give the pirraformnis a good stretch and need to practice these stretches every second day at first.
    I very keen to buy a traction device which you lay horizontally on and the hips are fixed in position with a wheel pulls the upper body.
    I know resting my arms along a benchtop with my body being in the 90 degree angle and suspending my body weight. That stretch is an awesome relief.
    One issue I have is my fused left elbow and the difficulty I have putting weight on it. Definitely cannot put any substantial weight on my left palm or even my left wrist/forearm, but once the upper arm takes the weight it’s fine.
    I saw that traction machine the other day and promised myself once our finances are on track again, I will be easily be able to buy it.
    It looks like the perfect way to safely stretch.
    In the meantime, I’m going to do the stretches you just did as best as I can. Thanks for these practical tips as the nerve pain has been horrific at times.
    Even just stretching the piraformis either side has helped a bit.

    This guy speaks the truth. Good stuff. Hey doc - my piriformis has gotten healthier through Pilates core work and posture correction but my left quads are still super tight. Can you recommend anything? I do the coach stretch but it doesn’t seem to get anywhere and if I push it I aggravate my knee. Thank you for the video btw

    Smartest explanation I have found so far . You remind me of Dr Bergman . I am currently having a strange issue , started with pain in my hamstrings then numb left leg and feet , like it suddenly drops without me doing anything then pulsating burning and tingling feeling from hamstrings to calf to bottom of my feet like it is on fire and between my butt check is on fire. , this happens mainly of I seat on a chair for 20 min , doesn't happen much on a soft sofa , I have diagnosed myself as sciatic nerve pinch , but I don't know if it is back or somewhere in my hip/upper hamstring caused by sitting wrong and seat without good cushion for long periods of time.

    I am in pain for the last 11 months. The pain is on the left hip and also on the lower back horizontally. I have early morning stiffness, pain worsens when sitting more that 20 min, and after I take a walk, I get tired easily, cannot stand up too long. Dr put me on muscle rlx and gabapentin for pinched nerve but those making me suicidal. I have the impression that it will never go away. I am 43 but I feel older and weaker than my dad. I cannot even do yard work for half an hour. So far meds did not help other than relieving my pain little bit.

    This is the best video I’ve seen. Thank you so much. My left piriformis gets so tight, I stretch it and get relief for 15 minutes to an hour and it’s back. It gets way worse with sitting. Standing and walking helps. Extensions don’t seem to help either so now I’m confused.

    Hi. Great video. I have pain in left buttock for years. I have still had months of no real bother. Could still run sub 19 5k. But I get sitbone and upper hamstring pain now. That's what has stopped me running for last 10 weeks. I'm lost what to do. Had MRI scan 2 years ago. No results of a anything.

    I've been experiencing sharp pains in my upper, hip/butt during long shifts as a nurses' aide. The long durations of walking trigger it, and stretching the hip tends to relieve the pain for a brief time, but it always comes back. It gets to the point where a leg just collapses. When I'm not at work I don't have problems. I'm not overweight and have always been relatively active. No problems showed up in an x-ray. Do you think this video or any of your other videos might apply to my situation?

    When I do the stretch on my back that stretches the hip joint it feels better. However, when I sit down or stand it is intense pain. More intense from prolonged standing than driving in a sitting position. There is no pain going down my leg yet just on the left side on the piriformis area. Thanks!

    Hello and thanks for the video. I am a 38 year old male who had a spinal surgery at age 17, a fusion of my spine to help correct a 57 degree scoliosis curve with Harrington rods. Beneath the fusion I had a herniated disc at about L4. Fast forward a few years, I’m at a lean body weight and weight train regularly with some sciatic pain but overall was doing ok with almost all exercises. All of a sudden I started getting a lot of left glute pain, which I think may be peraformis, and intense left nerve sciatica you the foot. The only thing I can think of is the leg press I’ve been doing once a week pretty consistently the past 6 months. It’s pretty painful and I can barely walk a mile before having to sit down from the pain. Should I just keep stretching or do you think cortisone shots could have a benefit to my nerve? The pain is pretty bad. I realize I am a stubborn guy, and don’t want to stop weight training, but I admit to you I have been pressing a lot of weight on the leg press machine. NO squats though just the machine. Should I stop leg press also? Thank you!

    I'm 75, still very active... but my piriformis on the right side is driving me nuts! I've been to PT and use a tennis ball regularly. Your stretch definitely helps; almost the opposite of the therapy I've been exposed to. So thanks for that. Just one more thing: the spasm is always in the morning and subsides after about an hour or so. But it is intense, really intense pain. Any thoughts? Once again, thank you; you're a God send...

    I like what you’re saying I’ve had piriformis syndrome I said that with parentheses her at seven weeks and it burns hurts just had an MRI of my lumbar and I think it’s L4 and L5 then tractor doctor on the 10th I go to sprouts and we have in Boston Massachusetts

    Thank you for the effort to get info out for people to help themselves. I did a 2 yr program online in 8 months, which I began walking like a lil ole lady bent over. Took me along time to correct, until I injured knee when getting out of chair circling around back of chair. Swelling in knee up leg to back in gluten and lower back. Because of swelling and pain Dr said nerve in leg is being bothered which the 3 pressure points confirmed that: one in gluten which I still have and 2 in lower back left of spine which went away during medication treatment. Symptoms I do have is the gluten pressure point and the hip and leg joint doesn't flow when walking like my other leg and tightening in lower back.Could this be a sign of this syndrome. Would like to hear what you say. Thank you 😊

What gets me it's the pressure point I can feel in my buttocks and pulling I feel in the leg and joint area, inner thigh, when walking and when I raise myself from a sitting position. But that pressure point can be done one moment and another irritating cause it will interfere with mobility. What do you think?

    I'm stuck in bed due to severe hip/glute pain when I stand, sit, and walking is almost impossible. The searing pain is in the hip/glute area, no resonating pain down my right leg. Started 2 mornings ago. Ibuprofen nor hydrocodone helped. I'm thinking pinched nerve in my hip. What do you think?

    I have a question... But first...
    I haven’t had an issue with the nerve in my left leg (due a back surgery years ago) which really effects my left foot so badly, in over a year. Happened 2 days and it was because of sitting down for 3 hours doing homework! UGH! Now, I know NOT what to do when it gets better. But my question, since this is affecting my left foot, (back heel mainly. Feel like there is a hole in my heel. And the calf muscle slightly as well.) it has to be stemming from my back, right? I did feel a slight tingling in my lower back as it began happening down my leg into my foot (left side - L5/S1) I don’t want to make symptoms worse. Rest, Ice, heat, Curamin (anti-inflammatory) and massaging my leg helps a bunch. But I do want this to go away a.s.a.p. so I can get back to class next week. Any suggestions? Thank you so much!
    P.S. God I wish I could speak to you, too. Like over the phone. Lol. The doctors here where I live are no help, whatsoever. Just want to feed me pills. Very bad. It seems like, individuals such as yourself, who make UA-cam videos about situations like this, seem like you really want to help the person out whom is having the issue. That’s just my opinion.     I self-diagnosed piriformis syndrome seven years ago and was doing low weight romanian deadlift to get temporary relief. I just did the cobra with hips on the floor and got a relief that is significantly better than usual. So maybe the cause is somewhere in the lower back region. Thank you!

    Thank you doctor- my symptoms are different. I can start with numbness of toes while driving, or walking, no pain of the back. It is relieved with hip stretching but just returns. If I remember to palpate, deep in the pirformis area to locate a very sensitive deep in the gluten area both hips. In the morning I am guaranteed to have leg tightness until I walk around a bit. During work hours I will experience foot aches along with numbness/tightness like a vice compressing my foot each day. I do not have a desk job, RN up on legs all day. Now with increased body awareness I can feel my hamstrings getting tight then numbness will for sure follow along with lower back tightness. With all this the foot/toes compression is the worse. Can you guide me on restorative exercises plan please.

    I'd love to get assessed by you. I've had chronic pain in the piriformis area for 7-8 years. Doctors thought it was a spur + tearing in my hip so I had hip surgery. The pain remained.
    I have pain 24/7 and worsens with sitting, walking, standing, running - basically anything. When it flares up, I have to sleep.
    The side effect from this is my general fitness and body condition has plummeted. I've lost nearly all my muscle tone and I'm weak. Combined with that problem, I also have impingement in both shoulders, which renders my arms pretty useless.
    I've been unemployed the whole time that I've had these problems, as I basically can't function normally. I've tried a couple of UA-cam rehab programs but nothing has helped. The pain always stops me from progressing.

    I get really bad, sharp pain when I lift my bad side's leg above my head while lying on my back. The pain radiates throughout my uppermost glute area, from the sacrum to the hip. The cobra pose makes that pain in the upper glute feel a really sharp pain as well. Do you think this is a glute issue, or do you think it stems from a herniated disc? I've seen several doctors and no one can provide a consensus diagnosis of what the main issue is

      Without a doubt a thorough 45 minute examination in-person would give us that answer to that. Yet without being able to tinker with things in person it would be a educated guess. Most people under the age of 40 it tends to be a disc. If I had to estimate under 10% of the time is it ever a glute issue. Yet much of the time it can be from nerve pressure (that lumbar extension can make worse temporarily). The best thing to do is just email me and we can talk there.     Okay, You lost me at the “ common denominator “ moment.....when I sit in a chair, bend over to tie my shoes, etc....I Huston my lower back and then when I lie down to ease my pain, it hurts in my buttock, groin and down my leg. But I have been through therapy for the Piriformis before and when I put my hand or fist under it while laying down I could faint with the new intensity of pain. So I’m trying to learn what you mean by the common denominator...that exercise you show facing down with the back extension is fine for about a minute before my back kicks in. what should I be doing to help this? Thank you sooooooo much! 🙏💕

    As I understand it Piriformis Syndrome is caused by the piriformis muscle squeezing the sciatic nerve which causes pain tingling etc down the leg and often the foot (as in my cases). It does not have any connection with spinal defects. If the cause originates in the spine//lower back it is Sciatica.

    When I lay on my stomach up on my elbows the pain surges in my piriformis, plus I get immediate tingling in my calf and foot (laterally). My pain seems to be caused by standing and pivoting for an extended period in the kitchen, or reclining and occasionally sitting. Sometimes sitting brings relief, even for long periods of time, although even then occasionally when I stand up, tingling occurs in the calf. Standing to talk to people and shopping often become unbearable, but walking usually brings immediate relief from all symptoms within less than a half a minute--unless the piriformis is overstrained. Does my piriformis need to be stretched or loosened? Could this be caused by falling backwards and landing flat on my back injuring my sacrum about 6 years ago?

    could a tight pirifomis/pinched nerve there cause numbness in the knee area? I only get numbness when i wear compression leggings and nothing is helping

      Sure can. Numbness and tingling is almost never a tight muscle. They are symptoms of nerves according to most literature.     I had hip replacement one year ago...now, I have piriformis syndrome that I was studying and trying to relieve...and it now has effected my sciatic nerve and I feel pain down to the back of my knee. I learned that sometimes it doesn't need stretching...it might need contracting instead. My thoughts are that the surgery to replace the hip has caused an underlying condition...and I cannot seem to find relief. Your thoughts.

    My issue came on suddenly when I did a corkscrew stretch on my side. I've done them for decades, and it always mobilized me and made me feel better (Sometimes it would pop, sometimes not), but when I did it Monday morning, I heard multiple pops and a shot of pain down my sciatic nerve. Now I am unable to sit, and am in a great deal of pain sitting. If I lay on the painful side in my bed, pain reduces some, or if I'm laying in my recliner. Also, if I pres down and hold the bottom of my glute muscle, it can make the pain go away. Does this sound like a temporary muscle injury to you? The o;nly difference this time, was my glute muscles were EXTRMELY sore due to activity the day before when this hapened.

    My pain comes and goes but happens when I lift my leg like getting into a car. I can mountain bike all day without pain but the simple movement of lifting that knee up brings on the pain right in the piriformis area? thanks !

    Got to this video while searching for how to solve this pain. It’s been consistent for few weeks now and doesn’t seem to go away even with many hip and back stretches which got me to think that it may be disc related. I’ve been trying to use stretches and strengthening every morning while continuing my regular weight and jiujitsu training. I also drive a lot. usuallly my low back is stiff after grappling. Should I stop training? Would this stretch you demonstrated help heal disc issues? Thanks

      Healing disc comes in a few steps. The stretch we showed tends to chill the disc out but it does not build support for it to be challenged by loads. We cover the support aspect in our courses.
