I think Juliet was truly scared of her love for James. She loved him so much that she couldn't imagine life without him. But really concerned her in this scene is that James was willing to let everyone else die so she could live. Like he was willing to get rid of anyone who tried to get between them.
You could tell that at this point Juliet's motivations going back to the island to stop Jack were still in the dark. She says they can't let other people to die but was it actually her real reason? I think she agreed to it because she knew there was a real chance for her to die. At this point, Juliet considered dying less painful than losing James. She would rather die than be alive and possibly lose James.
