    Hello Dr.
    Thank you for sharing the great tips and knowledge.
    I am wondering if the jaw sets are any good to practice the exodontia as I don't have access to patients.( I passed my dental school and haven't entered the workforce yet )
    I have a home setup where I can practice.

      Aparana - thanks for reaching out. I would encourage you to seek out as much information as you can regarding exodontia at this point in time. Having a typodont to practice on really won't do you much good as it does not simulate real life very well. Suturing can be practiced at home and you can purchase suturing pads to practice on if you choose. That's my two cents. All the best!

    Hey Doc, I was wondering if you could give your thoughts on antibiotics. Say a patient comes in with a small 2 mm in diameter abscess and the only option for that patient is extraction (or maybe an I and D), would you extract the same day or prescribe a course of antibiotics first?

      The standard of care is to extract the tooth. It’s pretty rare that you wouldn’t be able to get the patient comfortable enough to remove the tooth due to the infection that is present even though perhaps it can reduce the effectiveness of the anesthetic in that region. Use blocks when possible and supplement them with PDL injections. As for antibiotics you have no idea what you’re treating so you’re just giving them hoping they will work. In the meantime if they don’t work, you’re just allowing the infection to get worse. So whenever possible remove the tooth as soon as possible. If you feel antibiotics are needed due to the patient’s immune status, health history, clinical presentation, etc. you should then support their healing with antibiotics following the extraction and do an I and D again if you feel it is necessary. The main focus though is to eliminate the source of infection which is the offending tooth. Hope that helps! All the best!

      @@OnlineExodontia thank you, im amazed at all these dentists out there so hell bent on prescribing antibiotics any chance they can get for fear of causing post-operative complications by immediately extracting the tooth, which the literature shows are exceedingly rare. so many misinformed practitioners out there. not to mention all the possible negative consequences of rampant antibiotic prescription without any justification!

      I would premedicate with 1 g amoxicillin 30 mins prior to extraction. Post-extraction, I would currette the socket till I see clean shiny glistening bone margins and then irrigate sicket with betadine-saline. Metrogyl to be added in postop phase of course. I and D if needed.
