As long as you have the recipe, the clothes is in obvious good condition (the condition you bought it and tag included), and returned within the required time, they really don’t ask or if they ask anything if it’s a size thing and if you’d like the same thing but different size or depending on the store when they give a refund, it’s in the form of a gift card ^^; so you still need to buy something from the store.
Yes, some stores do ask you why. It could be policy. But, I doubt the answer would determine whether or not you get a refund. It's probably just protocol.
