If, Corneal thickness: 500 µm Power: -8.00 Sph Cap thickness: 110 µm Optic Zone: 6.5 mm Estimated ablation(15µm/D): 120µm What would be the actual RSB ? 500 - (110 + 120) = 270 µm or, 500 - (60* + 120) = 320 µm * Epithelium thickness (approx.) More than 300 µm RSB is preferred by most doctors. So, does this surgery increase the risk of Ectasia ? Would you perform this ?
No I would not . Best I can do is to do your scotopic pupillometry as reduce optical zone accordingly and reduce cap thickness ( because I do caps of 90 micron usually ) . That should bring us to >300 RSBT and < 40% PTA . Otherwise , phakic lens is the only option

Sir i heard from one of my friend who underwent SMILE 3 Years ago...but now he have problem of halo and glare ...has this problem been improved in clear surgery
glare and haloes are mainly due to two reasons ( assuming that surgery was uncomplicated . 1. The size of the optical zone was smaller than required so light scatters at night time . 2. The centration of the treatment was improper leading to induction of Coma aberrations . These things are completely taken care of in CLEAR . However , during the recovery phase ( varying between 1-4 weeks ) everyone feels some amount of glare and haloes after any laser procedure . Persistence beyond this period is a problem and the reasons I hve listed above .

You can undergo Customized laser with c3r or icl or both depending on your spectacle power and corneal topography etc . Keratoconus treatment is not a “one size fits all” type of treatment .
