Cough reflex anatomy

  • The cough reflex is triggered by sensors in the airways and sent to the brainstem as part of the vagus nerves. Links in the brainstem between the solitary nucleus and probably the dorsal and ventral respiratory groups of nuclei send motor signals through the vagus nerves, phrenic nerves and various spinal nerves to the muscles of the larynx, diaphragm and body wall. The phases of the cough include the inspiratory phase when air is drawn in, the compression phase when the vocal cords are closed and pressure builds in the airways and then the expiratory phase when air is blown out of the airways at speed, hopefully expelling the irritant.

    As a person who enjoys classical music, I think chamber music triggers the cough reflex. The smaller the venue, the more coughing.

    Thank you.I love watching you anatomy lectures, they're so informative and easy to understand. I frequently get boughts of coughing especially when taking oral medication which I've been on since 2008. The worst aspiration I had was a round paracetamol tablet, it lodged in my airway and the coughing began, I was coughing so violently that I could feel my oesophagus snap shut at the end of each cough. This went on for around 5 minutes until I was able to expel the tablet which started off white now it was soaked in blood!
    I trigger this cough reflex about 5/6 times per month and always due to aspiration. My father died age 59 from 'Regurgitation of Stomach content inhaled into the lungs' this was caused by copious amounts of alcohol. I don't drink. Throught my work in the operating theatre as an anaesthetic nurse in ENT and many other specialities, I appreciate just how delicate the airway is especially the vocal cords.

    Now that you mention it, I notice if I successfully suppress a particularly bad cough, it ends up expressing itself as tearing up in the eyes. Wonder what's going on there...
