Sir I have been diagnosed with grade 3 sacroilitis in left si joint, sir I would like to know what happens in this grade and what precautions must be taken in grade 3.
Sir guide me.
Grade 3 only shows the long duration of the inflammatory process. Your AS would be treated based on its activity/ severity.
Sir I have bilateral sacrolitis with fused right hip joint pain due to ankylosing spondylitis.
Now the question is if I go for surgery then only hip joint replacement can not give me relief. And I don't want to fuse my sacrum with iliac bone to relief from sacrolitis because there will be more movement restrictions.
So what is advise according to you.?

Findings:
Minimal periarticular sclerosis seen at left sacro iliac joint.
Few punctate erosion / subchondral cystic changes seen at sacral aspect of bilateral sacro iliac joint. Heterogeneous fatty marrow changes are seen in sacrum and ilium.
The joint space is maintained.
No fluid is seen in the sacroiliac joint spaces.
Pre and paravertebral soft tissues appear normal.
Impression:

Sir recently I have gone for a MRI of sacroiliac joint and it shows complete bony ankylosis of bilateral sacroiliac joints with subchondral fat metaplasia and sclerosis features are sequel of chronic sacroilitis . No imaging evidence of active sacroilitis is seen. Sir can you explain this in more easy way .
You must have had sacroilitis but it is not very active as of now.

Sir my thoracic back pain started 5 months back , hlab27 test was positive and AS runs in our family. And doctor also diagnosed AS but sir as told by you in the first stage, inflammation will be seen in the sacroiliac joint but my MRI of sacro iliac joint was completely normal and also I don't have any pain associated with hip joint. I have this pain in one location that is in the slight right from the thoracic vertebrae ( where rib joins with the vertebrae)
So can AS also starts from mid back?

Yes. AS can start in the thoracic spine. In your case it could be affecting the joint where the rib attached to the vertebra.

As the CRP is high do not reduce the medications that you are taking even when there is no pain, continue seeing your rheumatologist. He would also look into other causes of high crp

Saaz chalu rakhne. Exercises nahi karte hoge to chalu karna. Agar saaz, exercise aur NSAIDs se kam nahi hua to biologics ya Tofacitinib ka sochna.

Doctor I've suffering from AS for 6 years. In december 2021 severe pain came suddenly in my left si joint. In the last month my doctor prescribed me tofacitinib 5 mg once daily ,saaz ds and methyprednisolone 4 mg and stopped nucoxia mr because my s.creatinine level was 1.7 & sgpt 51. Now my creatinine level and sgpt are normal. Doctor told me to take adalimumab but I can't take biologics due to financial problem so now he prescribed me tofacitinib twice daily and medrol 16 mg. Will tofacitinib work in my si joint pain ? I took tofacitinib 5mg once daily in the last month but it didn't work
Tofacitinib is an effective treatment for AS. In case it does not work you may have to think about biologics.

Sir mujhe kuch dino se hip mai bahut tez dard ho rha hai...chalne mai uthne mai chubne vala dard hota hai.... ekdum se uth bhi nhi pa rhi...na koi exercise kr pa rhi.... mujhe kya karna chahiye...pls help me 🙏

Etanercept accha injection hai; loge to accha rahega

SI joint ka injection hota hai magar usase accha Tofacitinib ya biologics hoga. Apne rheumatologist se Tofacitinib ya biologics ke baare me baat karna.

Sir mai AS ko face kar raha hun, 2016 ,2017 ,2018 mai mujhe 300mg infleximab raimicat biological liye tha | 2018,2019,2020,2021, mai bilkul thik rah parday 2-3 km running karta tha but 4-5 month Saaz n le saka kisi karan bs, jisse Feb 2022 se dikkat ho rahi hai | Sir biological injection fir se lena hoga ,body mai pain bahut hai pankiller lena pad raha hai sir pls guide me
Saaz aur NSAIDs se agar AS control me nahi aata hai to biologic lena padega
