Generally, you're first eligible to sign up for Medicare Part A and Part B starting 3 months before you turn 65 and ending 3 months after the month you turn 65. If you are not 65 yet then check if you are eligible for Medicaid or not. The only other option is getting employers sponsored insurance and if not working then buy insurance from online marketplace. Regarding second point as disadvantage of citizenship you have mentioned that American citizen person has to pay tax even if he is staying out of America ,so green card holders does not have to pay tax if he stays outside Us , needs clarification please

US citizen can live outside USA for many years and return whenever they want. When they live outside USA they are liable to pay US taxes. Green card holders have to be present in USA for at least 6 months in a year to keep their Permanent Residency status (Green Card Status) hence technically they are in USA regardless and has to pay taxes since they are in USA.
