I think it's more accurate to say he is lawful good, and he is just operating under a higher level of understanding. Then, we currently operate under. So, to us, he is evil, but if you had his understanding, you would see just how good he is
He's more Chaotic Good, he wants and will try to help you, but if you get in his way or if he thinks it's required, you are dead. Lawful Good would mean he will always follow the foundation, but he only follows them since they give him test subjects and because he finds it fun. The moment you piss him off, you are very likely to be "cured"

They operate under a different level of understanding then we do. I wouldn't say it's higher per se, more like a sidegrade, but different nonetheless. There is a creature in that known as Dajaal that perceives the passage and flow of time itself, able to see where the timelines branch and disperse in real time. He claims that normal humans would perceive it as insane, which is ludicrous to apply our sensibilities and understanding to it because it is beyond human, its consciousness and intelligence not being human but superhuman. Superman sees things far differently then that of the normal man, he can perceive radio signals and things invisible to the naked human eye. Thats why I believe the foundation hasn't been able to understand 042's sense of urgency and fear. Because they are thinking like humans, not like him. They don't see what it sees, they apply their level of understanding to it instead of trying to meet it at its level. To 042 explaining what the pestilence is to a foundation scientist is like trying to explain the common cold to a neural surgeon. To 042 the pestilence is common knowledge that everyone knows, asking about it is like looking at the dipshit with a runny nose and a bad cough asking why he is sick.
I think personally, Lawful Neutral is the best fit Lawful, he has his own code, morals, and rules, which he sticks very vehemently towards, however his actions are neither particularly evil or good. He does evil things of of, in his view, necessity or greater good.
I think also what attracts ppl to scp 49 is the fact that he seems so genuine in the way he talks and does things. As evil as they may seem he doesn't show any sort of sick desire or sadism in doing it. It's a nice change of pace for other scp's that are just straight evil or very animalistic. Similar vibe is scp-082 the cannibal tho the cannibal doesn't have the same altruistic goal he's just very well versed until he randomly eats you.
The thing is, you said it yourself: a lot of SCPS aren't just evil for the sake of evil. The plague doctor, in my opinion, has good motives. And that's based on a theory I made which includes SCP-2718 and SCP-5000, specifically The Entity. But that's just headcanon, anyway.

There are definitely modern SCPs that do stray from what the modern SCPs try to do. A great example is SCP-8000. I wont spoil anything, but ill say that 8000 is the definition of "write what you know" as it's more about personal struggles faced by the entry's writer Plague shown through really well written dialogue. I HIGHLY recommend reading it.
Personally what Ive seen a lot of SCPs go down the route of being more scientifically explained, almost lil extreme, to validate them. I mean the Living Tattoo is one example. They focus so much on how it functions, that leaves a lot of the mystery out of it. Or how they try to use other SCPs or tools to explain their existence like a multiverse. What makes SCP so cool IS the unknown. We know vaguely how it operates (not fully) and thats about it. The origins are what we ponder about. Plus the extremes the foundation goes to keep things under wraps helps emphasis the mystery.
Man still one of my favorite scps is scp 008-j “Geoff” It’s a probationary scp because they don’t know if it actually has powers or if it’s just a bunch of weird coincidences. Scp 008-j is just an unassuming guy named Geoff that casually gets lost and finds himself in scp facilities. No one knows if he has some kind of powers to end up there or if he just knows the code but the dude just randomly shows up asking for directions casually as hell frustrating the even living fuck out of an scp commander because they can’t lock the guy up because he gets out the same way he gets in Plus it gave us this golden transcript Commander Price: Alright people, it’s go time! I want suppressing fire on this thing NOW! Neptune squadron, hit it with everything you’ve got! If this thing gets one claw to the surface, then… SCP-008-J: Hey, I remember you! Commander Price: (Exasperated and enraged) FUCK! SCP-008-J: Can you help me out? I’m a bit turned around. I’m trying to get to Grays Street.
