Very confusing !! All I know is I lost my left kidney to cancer in 2014 and my remaining kidney has fallen to EGFR 49 possibly a little bit less and is going down so CKD3 possibly 4 so what does that mean to me aged 69 am I acute renal failure or am I ok and because my bladder also ruptured in the triple operation with a fissure between my colon and bladder and most of my sigmoid colon was taken out as well does that mean assuming I live to a ripe old age am I going to hit total failure and need dialysis or am I going to be okay on a kidney that is running on EGFR 30 or 40 ? Or how long has a piece of string ?

At 3:09 you mentioned that Intrarenal failure is the most common cause, but actually I've read a few different books that state that prerenal failure is the most common cause. the cause isn't 100% concluded as of yet, but the most common reason for this (that's being listed in the medical books) is that because of the high blood sugar level the basal membrane of the vessels (and thereby also the glomerus/glomeruli of the kidney) are being "glycolyzed", meaning that sugar sticks to proteins in the basal membrane of the vessels, causing them to thicken (thickened basal membrane) and therby narrowing the vessels. the vessels therefore are unable to perform their fuctions properly and certain organs are damaged, like the kidney, eyes or also nerves. hope this helps!

actually the most common cause is prerenal failure not intrarenal. Its 55% against 35 % just for the record. But great lecture as always ofcourse :)
And we are taught here that the prerenal is 70% and the intrarenal is 25%. :)
