Former health care worker. I quit well before retirement because the abuse from management and patients was just too much. I loved helping people but the entitlement and outright bullying and abuse wasn’t worth it. I’m slowing regaining my health and confidence again.
Same, after 26+ years with the same org. I left the cult, if you will. It took a long time to get out. And, like anything cultish, there is a high exit cost.❤

How do patients abuse you? Is it that they want too much care for their conditions (which is understandable at times) or are they insulting you when nothing wrong was done?

Only a Bible based world view can improve healthcare, b/c it says that individuals have intrinsic worth. Eg. coronary artery disease treated with a stent = 30K, with surgery = 150 K, with low fat vegan diet = 1k & provides by far the best results, but is almost never offered. Only love of the patient as a creature created in the image of God, could lead people to value the pt over the money. If atheistic Darwinism continues as the religion of the land, as it probably will, then health care will not improve, b/c it places higher value on money. To anyone who knows healthcare, this is obvious.

Nothing will stop the fall of America until we get rid of Corporations who monopolize everything, while the divide between rich and poor shrink. You’re a good man I can tell, because you care, however, don’t go down with the ship due to a country that cares about money more than you. Go work in a country where people need you, love you, and be so grateful for you. My husband is a doctor here in Egypt, and sometimes he takes a chicken instead of payment, or something pickled like vegetables, because his patients insist and they don’t have enough money. Doctors here are so down to earth, maybe it’s because they don’t get paid much, I don’t know, but I see the humanity.

As a stage 4 ovarian cancer patient the past 7 years and currently on chemo, I wouldn’t even know where to start. I have to deep breathe through my angst over lack of communication with my medical people. You’re opening a big fat can of worms. I’m here for it. Good luck!

You are a good guy, Doctor. My father, a Doctor, had a sign on the wall, in latin, that read "Don't let The Bastards get you down." I never knew how valuable that wisdom would become to me. I, myself, have endured a literal Hell on earth, mostly due to abuse and narcissism. My father was so wise. I miss him desperately, because I loved him so much, and he was also A Good Guy.. "Fight, Fight, Fight," as Trump said the day they tried to kill him. I took that to heart. We Need Good Guys like you.

I love what you are saying. I lived in Southeast Asia and now live in America. I think communities in southeast asia and came back to believe that people are generally happier in Southeast Asia. Dear Chad. More and more doctors comes forward and tells almost the same story. To me as a human being and patient, I have met a system, that was more than loyal to big pharma and their collagues, than to the health of their patients. Most doctors seems to be trapped in the same system as their patients, but obviously havent succeeded changing much.
My experience is that Its impressive what a patient with any systemic disorder, can achieve by introducing intermittent fasting, better mouth hygiene, and gradually learn how to avoid industrial foods, and those type of medications, that were only made for profit. Its obvious to me, that smoking, alkohol, overweight and ignorance of own influence, are the main killers of quality of life and life itself. -anyway its just a thought. Per in Denmark. 😎🇩🇰

Hello, I had the same feelings watching Sr. Doobies videos and it's nice to have such authenticity from you two on UA-cam. Haven't seen that side from other docs on the platform prior.
I am a family medicine resident with UA-cam experience, thinking about starting my own series taking about my experiences, partially as my own therapy. The educational system the last few years has been particularly tough on me, and I've wanted to take a long break since starting. Interested in underserved and travel medicine, but think i will need a break and to pay off my debts prior to going moreso into the things i want to do since they do not pay as well as far as i know

Thanks for your insights, and for all you do for your patients. It's not easy to change our healthcare system, but I'm glad you have that goal. I hope you will make positive changes. In texas, the corrupt governor has refused federal Medicaid funding for the last10 years, which was intended to supplement ACA healthcare for Texans. It's political, to create a false narrative that ACA is not good. The net result is that hundreds of rural clinics and hospitals in texas have been shut down. We really need physicians to speak out in texas. What the governor has done is very shameful, and harmful to our people in texas.

@@prof_writer thank you. The majority of the determinants of health, as you may know, are social determinants. So your background and knowledge and experience with the social sciences is important and appreciated.
I have a similar journey. Including a MPH. Sadly providers of direct patient care in US have become replaceable commodities. We haven’t fully realized this yet. I would have NEVER said this before, but I now believe collective bargaining is the only way we can save our profession. It will save our patients too.

Well done. I’m an 85-year-old woman surviving the current medical system by doing my research and changing providers to a more holistic practice which is growing, thankfully. I’ve found a situation where time is taken with me, where I can actually get an appointment or speak to my care team any time. I hope they can survive this insanity.

@@drchadswanson Certainly. It is One Medical for Seniors. When I first started it was called Iora but was bought by Amazon and the name was changed. That was 2 years ago and so far it hasn’t changed except for now having 24/7 access by phone. Iora was first started by a Dartmouth grad who wanted to change how medicine was done. Sadly it is only available for people on Medicare. I have never received a bill. Happy to answer any questions.

I've watched Dr. Goobie's videos and they're very relevant. Integrative medicine considers and applies the importance of looking at the whole patient and not just his/her medical diagnosis with good outcomes. I've had some wonderful experiences working with doctors who use this model. We are more than our bodies!! One technical comment that I thought might be helpful to you. Your sharing is really valuable and adds to this conversation. There are so many variables with the health care system, not the least of which is Big Pharma and insurance companies. We owned three community pharmacies for 50 years and were forced out by both of these greedy behemoths. We just couldn't compete. They're calling the shots and ruining health care in so many ways!!

I'm a capitalist. I love America and the social mobility it provides. However, I think healthcare should really be nationalized. Of course doctors can still provide additional private services on top of it (its a free country after all). But I believe that basic healthcare should be nationalized. The reason is because, if you think about it, we are already doing that. But right now we are only nationalizing the consequences of a bad healthcare system. When someone cannot afford healthcare, their ability to work suffers, their finances suffer, their family suffers, and other people around them are affected negatively. The society is already absorbing that cost. It is better that we try to keep as many Americans as healthy as possible by having an affordable healthcare for anyone, employed or not. It is just cheaper overall. (As a system!)

I’m not so sure if nationalizing our health system is politically feasible, so I’m in favor of free, basic, universal health insurance, that doesn’t necessarily have to be nationalized. There is this silience code- can’t talk about the distinctions of the insurances - but the staticians are all assisting if the investors of the insurance companies so they can make $$… so that is why you have all the pre auths, the # of PT- before you can get surgeries and sooo much more… it’s the basic “ wager “ mentality.

My son he is 10 , since he was he wants to be come a surgery doctor . He only focused on that idea he wants to do operation in heart lung abdomen . Watching these videos I constsntly having conversation about challenges in medical field ,. After his grandpa had open heart surgery it got to his mind he needs to save people .

i had to leave my last job, the private equity "partner" agilon would serve you with their data mining forms every morning for the days pts. we were admonished to upcode, upcode, upcode and if we didnt we were called on thr carpet. now no one actually cared if you listened to the actual patient or did thr right thing for them...they only wanted the higher billing codes in the note. shameful

I have family in healthcare doll. Doctors, nurses and PA's. The stories I hear are nefarious and horrific. Nothing is shocking to me anymore. The insurance companies run the show. Yes, there are some good doctors , nurses and health care workers. It's all about profit over people. Two of my sisters quit nursing because 90% of their job was paperwork and they could not get to their patients. Many people die in hospital because there is a shortage of "employees."

Like Dr Goobie said that some of his patients complaints about their pain but then they had to cancel the surgery because the pain had gone away! Why? Bc the pain could be due to stress in their personal life, job, etc. and Doctor should ask patients about that. I used to have a primary physician who would ask me questions about my life, what I eat etc. but then he retired . He practiced solo. Young doctors don’t have such luxury. They got bought out- no choice, and they work like maniacs. I love you doctors!
I had 2 herniated discs in thoracic area. Before surgery the pain disipated. Sometimes this happens. It still aches now and then though.

@@lindag1372 That's fantastic to know. Was your thoracic area otherwise healthy and without moderate or severe degeneration and other issues? I do think lifestyle, diet and stress management helps, but it seems to not be the major factor when it comes to the spine.

This is a loaded question- how does one balance their homeostasis? What are some things one should consider when trying to balance their lives? nutrition, input v output, mental health, social health, medications, where to start if you feel out of balanced or not functioning as well as you could… sorry not sure if I asked it right but it would be cool to hear your thoughts
That’s a great question, an important question. Of course, it depends on where you are. I think that is helpful to think of balancing our lives as a journey. A few concrete suggestions: first, connect with others, walk the journey with loved ones. Family, friends, etc. Two. Get outside and move. Gone walks, hikes. Three. Learn. Then continue the journey. What are some of your thoughts? Was that helpful at all?
I got injured working out - it's not horrific, but I've had persistent pain for over a year now. I felt like I was in a flowchart and that too much was focused on statistics rather than who I was and what happened. For example, I explained that the exercise placed an enormous amount of stress on the thoracic spine and it was completely dismissed even without imaging. I did imaging separately and it turned out that I had bulges and a herniation (no neurological compromise though). I still have pain in my mid back and I've seen some try to suggest that those issues may have existed before that. I find it beyond ridiculous. I know how I felt and moved. There was no stinging or deep ache and nor was there any issue with walking or standing for extended periods. I could bend and rotate any way I liked with no stiffness or pain.

Healthcare is to expensive. I consider it a rip off. I'm 61 and in good health. Insurance wants $950-$1200 a month for I've been self employed. It's just so overpriced for most everything. America and it's greed.I may be moving to another country. The rich and famous can have this country full of hate and division. Greed here is good. I wish this to change.

I left, I feel the same way you do. I live in Egypt and I’m 60 now, been here 18 years. I have heart failure now, and when I end up in ICU for 4 days it cost me $2,000 for everything. So please tell me why Americans can’t afford healthcare? Greed, pure greed!
@Fatma-mx6cc take care of yourself. America is set up for the wealthy. The rest that work hard pay for most everything. The poor who use the system get free handouts. I may move, cause I don't enjoy all the greed and division, hate. I like where I live, but the atmosphere of hate is hard to be around.
I do think greed plays a significant, major role. But it’s not everything. They were good hearted people who get stuck in the system, have other worries and concerns, and do their job.

Thanks for stopping by. I’m certainly in favor of an ED thoracotomy in the rare instance that it indicated. My focus here on UA-cam isn’t on detailed clinical issues, but larger systems and societal issues. There are plenty of videos about economies though. Thanks.

Doc, the entire system needs and overhaul starting with diagnostics, which physicians really suck at. We have computers, that when combined with a technician taking orders from it and administering drugs and other stimuli to elicit computer analysis could reduce diagnosis from months to years and maybe decades, to an hour or two.
Thanks, John Smith. We need our vision and incentives to align with what we really want. Until that happens, agreed, that diagnostics and many other aspects of our health and healthcare willbe sub optimal. Thanks again!

One reason for these issues is doctors allow themselves to be beholden to insurance companies. But hey, $$ talks. And they get you by burdening students with years of house mortgage-sized student loans. And most doctors are afraid to think outside the box because of medicolegal risks. So more of following the herd. Yay🎉

I'd like to hear your thoughts on healthcare IT. How do IT systems affect patient care both positively and negatively. What are some areas for improvement in the future?
Thank you. I’ll give this more thought. But here are some initial reactions: 1. AI is already here, institutions, professional, and organizational norms and requirements. Need to be much more nimble and adaptive to keep up with AI. 2. Electronic medical records have been both a blessing and a curse. 3. Security is obviously a huge issue and I’m not qualified to comment on it much. 3. Interoperability, the ability to obtain medical records from other healthcare systems is a huge issue. Big topic, one that I don’t have much experience with or knowledge about, but I’ll learn more. What are your thoughts?
@@drchadswanson I'm going to weigh in if I may. Some background - Been a computer guy in both travel and supply chain management, taught a few years in a University and introduced students to Neural Nets, and used them myself in industry. Recently been a patient, with three surgeries in the past three years. So...
I think AI will be huge in medicine. Radiology, Pathology, and Internal Medicine seem like the most obvious specialties for the first big inroads. If it doesn't outright replace doctors AI can be a huge help for physicians.
Electronic records are nice for patients because you can see the result of tests right away.. Good for doctors so they don't have to call everyone to tell them their triglycerides are ok - or whatever. Bad for patients because the jargon can be terrifying...but also good because you can track your own metabolic progress for example. In Honolulu, the two big hospital systems use the same electronic records, and the patient has the option to link them together so doctors in both systems can see your records. I think this is a huge benefit. Security is important and I'm not an expert on that.
Anyway, just a few thoughts.
The business elements of American health care are so out of synch with true health care.

If the definition of 'patient centered care' means being ignored, then I'm receiving awesome nothing. Its at the university hospital in the state. My butt checks are more patient centered. Im in Social Security Disability, with multiple medical challenges. Included tapering off long term high dose benzos. Thats all they see. Not a human being, just "shes not taking her 'medication'. I've been sedated for long enough - 40 years too much. I'm still breathing. Hello, I'm glad i can put a sentence together. And usually comprehend. Any legal help???

Thank you. It is interesting and beneficial to live in a world where we can connect with people that are having similar experiences. Two things come to mind when I think of what I learned from early community base models: first, that a relentless focus on Health can bring us down Paz that we might not of think thought of. New institutions, political, advocacy, etc., etc. Second, human relationships and gatherings are crucial for health. I’ll make a video about this!

i still remember in 2021 when i was already waiting 8-10 hours into the night waiting for them to green light the surgery to cut into my stomach to take something out and all the doctor was concerned about was getting me my covid-19 vaccinations before the surgery, like wtf?
Covid was a tough time, that’s for sure, on lots of levels. We could’ve done much better.

Amazing dentures not covered by Medicare . Is this a direct reflection of not caring about poor people? Chose one specific example of insuranice and income inequality..... Thank you very much for your suggestions. I will definitely be posting about insurance. However, I think it’s important for us all to realize and remember that the clinical medicine services covered by insurance only contribute to about 5 to 20% of health status that we can control.
Western Medicine is a joke. When my husband was undergoing treatments for Stage 4 Colorectal Cancer we were so desperate for direction in terms of things like “what kind of foods to avoid/eat” , and NO supplements were allowed while undergoing chemotherapy. They will ONLY prescribe whatever the standard treatment is, and it hasn’t changed in 60yrs. They won’t offer any opinion of their own- even when you know they know- you know? 😆

Keep looking there are a few “unicorn “ doctors out there. I’m on my 3rd one at the same facility & he might be a keeper. He actually encourages communicating through the portal instead of making an appointment every time you have a question or an update.

In the beginning of your video i though, that it will be some kind critic to Dr. Gooby... But it is not. Which is good. In your short explanation, i can see, that you are in the stress just now. Your mouth is dry, you talking a bit fast au usual.. In the end of your video you said, that your promise that you will answer the questions daily.. But you know what i am thinking.. Try to find your own balance first. 🙏 I think, your own resource is close to give any kind of information to somebody else. You have to come down. To find your harmony. For me was very helpful meditate with program of Dr. Deepak Chopra. After some days i started to feel very different than ever before.. I understood a lot of question what i was thinking quite a wile.. It start some kind of automatically.. Very special.. I did it just15 min / day.. Anyway.. Just because someone’s mouth is dry does not mean they’re unable to help others. I’m glad that you found your peace with Chopra and i hope you can appreciate that “knowing the truth” is much easier than seeking it… this was a very brief video and it’s unlikely that you’d know what this person could or couldn’t offer to others …

Most of the clinical people I have dealt with I believe to be truly caring people, but there is sense among doctors in particular that they have worked so hard to earn their skills that they are going to get their piece of the pie. Even though the pie is way too big

You have about 0% chance of changing the healthcare system. It's locked up in a top down, centralized, data driven, algorithmic, cookbook care. The art is gone
You might be (almost) right. The chance might be very low, approaching zero, but I don’t think it’s zero. I am energized by successful social movements of the past: abolition of slavery, child labor laws, public education, women’s suffrage, spread of democracy, LGBTQ, etc., etc. People have thought that lots of developments that occurred weren’t possible.
@@drchadswanson A lot of people with covid saw the fraud and are switching to alternative medicine. Thank you for being so frank. So many good dr. lost their licence because they refused to follow the hospital protocols.

Here are my initial thoughts: first, I wonder how much can be explained by a nationwide difference in incomes from all sources. Two. Financial gain has always been a major driver in American medicine, the profession has been aggressive in organizational and political moves to decrease competition and ensure a high income. Three. Physicians would argue that they sacrifice a tremendous amount of their lives in training to, and that the high income is much deserved. As a practicing emergency physician, I can certainly relate to that. Four. There are no active healthy market forces That would reward organizations that provide better services at lower cost. What are your thoughts? More to come…
@@drchadswansonin regards to point 3 (CRNA here), the majority of the time and money spent in getting to where you want to be is unnecessary. Think of all the unnecessary college classes. I trained with “certificate CRNAs” (nurses trained by hospitals) who are very knowledgeable, and now a PhD is required to do the same job. Think about that.
@@ts3063 thank you. You’re absolutely right, I agree 100%. The educational requirements to be a physician are largely 100 years old, with many requirements that make no sense. And so there’s unnecessary time not earning income that many physicians would argue justifies a higher income.
