You forgot the opening narrations of Crime & Punishment and maybe True Crime if it had one. Also, Los Angeles' opening starts off with "In the city of Los Angeles...."
There is one episode from season 1 titled The Torrents of Greed Part 2 where the opening narration was different, the episode where Ben Stone was going after a mob capo and the mob boss tried to use the system to frame Stone
