I'm about to reply 6 years later but to be fair Cox does hate everyone besides Carla and his kids. But deep down he could tolerate some of the characters like Elliot.

Carla likes and is close with, Jordan loves but doesn’t show it to much, Turk has a competitive respectful relationship, Kelso and him become bud in the last season, Janitor I’d say he is cool with like a guy you get beers with, Elliott he was always her mentor deep down and helped her when she needed it, JD is obvious he explained his feelings about him. But a stranger can’t just come up and expect to befriend him that’s not his character. It’s a mask to hide his true feelings about the main characters but it’s not a mask that hides his feelings about other people. He is still a hard exterior guy that cares for the characters listed but doesn’t care that much about strangers.
You forgot to mention his patients. Cox cares deeply about his patients. Perhaps even too deeply. When something goes wrong and he thinks it was his fault, he feels massive guilt. One time, it even bothered him so much that he quit because he just couldn't take it anymore and it took all of his friends combined to bring him back. Like JD once said: 'Even after all these years when something goes bad, it still hits you this hard. That's the kind of doctor I want to be.'
Keith was the only good recurring cast member from the younger generation. They should have given him more focus in season 9 than a bunch of those newbies.
I believe it is the episode where Elliot tells Carla that Keith had broken up with her, to hide the fact that she knew Carla was pregnant. I can't recall why Elliot had crossed Cox, but his game was getting Carla to emotionally hurt Keith and therefore Elliot, since they are a couple.
