San Francisco
July 6th, 2013
A Boeing triple seven carrying over 300 people begins sinking towards the water, on its final approach to San Francisco. The passengers are oblivious to the danger they are in, but the pilots are confused about why their aircraft isn’t responding. Horrified onlookers watch as the massive plane desperately tries to climb. The captain slams the throttle levers fully forwards, but has he done this soon enough to save the plane?
This is the story of Asiana Airlines flight 214.

this is tragic but also embarrassing that with 4 pilots in the cockpit, they messed up in perfect landing conditions
The monitoring pilot kept issuing callouts about speed and sink rate.
It’s wild just how many air accidents are pilot error.
Yeah, it's a scary thought that no one was dialed in. *I think there were only 3 pilots in the cockpit at the time of the crash. (9:08) The 4th was in the cabin & I'll bet he was the one of the 4 who was injured.
@@Nick_Reece Seems this is more of a lesson in how easy it can be to become complacent--both pilots were quite experienced, even on other planes (i.e. not the 777), but still were able to make basic errors in judgment; this is a theme common in not online commercial airline accidents, but basically any accident involving complex systems. Moreover, as this video showed, one of those errors was actually because of the pilot's experience with the Airbus and lack of experience with the Boeing--his instinct upon approach was to pitch the nose up instead of pushing the throttle because that would have worked on the Airbus that he was accustomed to.
@@Nick_Reecewell as mentioned in the video, the glide slope wasn't working
I'm honestly amazed by how easily this situation could've been avoided.
And to think these are TRAINED pilots on board ignoring basic protocol
I’m not a pilot yet I was screaming at my screen for them to go around miles before they even realised what the hell was going on. And even then they didn’t realise what was going on until they were 100ft above the ground. Beyond comprehension
How do such experienced pilots make such bad decisions?
@CrazyPanda688 relying too much on automation probably did it
Some seniors have a mindset of letting the trainee do the work and them watching and observing.
Great video. The graphics get better each time!
Great video, about the crash that became a literal meme.

Not emphasizing your pilots skills to actually fly the plane is so wild to me
21:01 aviation equivalence of 'bruh'
pitching up to increase speed... damn. totally relying on the automated system to spool up the engines when pitched up.
My goodness, your editing skills have gotten so much better than a year ago! You will continue to be my favorite aviation channel. Bravo!

@@GreenDotAviation To be honest, there probably isnt a person who dislikes your editing style. Its just so good
Me listening to pretty much all of the videos on the background: Yes, I agree
These pilots managed to crash a perfectly functional aircraft short of the runway during the day, in perfect weather and with no wind issues. Absolutely unforgivable airmanship.
There are probably 2 unemployed more😂 Luck for the captain, if he does not go to prison
"pilots" lol, more like button mashers
@@Passions how do you mean button mashers?
@@Aviation_guy sorry to say, asiana also played role here, as they enforced automation instead of airmanship... so they made their crews more as "button mashers" and not pilots that fly the airplanes...
@@kirjuschaks Both pilots returned to work within a week, as ground staff. They are probably retired by now.
Its just unexplainable how the crew didnt choose to do a go around here. Especially at 500 feet, but also before this point.
Get-there-itis might’ve played a small factor here
They didn't know how because Asiana didn't teach them. They were taught to use automation at almost all times. They now know how to FLY the plane but not then and it was per policy
If you're too high, you get another try.
If you're too low, you don't get another go.
When you are tired , your brain stops working. The student pilot getting his certification would have probably failed and the training pilot was not doing his job either. Who knows, why. The third guy was probably tired also and maybe didn't want to speak up and it all happened too quickly. So maybe it is a cultural thing where no one wants to rock the boat.
thats what happens when a company makes their employees so insecure of their abilities to the point that they blackout and dont know what to do.
"the crew were high and fast, the last place you'd want to be on any approach!", i'd say low and slow is the last place
Absolutely true
At that point? Not really. Low and slow is solved by increasing speed for a bit. High and fast is a problem

@@RAAFLightning1: I believe you!
I remember this crash. I flew into SanFran the day before with Delta. As we were landing, we suddenly went back up into the air. It freaked me out! I can't remember who he was anymore but the guy next to me had experience with planes or something... I guess he noticed I was confused and he actually said to me that the runway was doing some construction/repairs and he just explained a bunch of stuff about how planes land and why it might have taken back up suddenly and why were were fine. The next day, I woke up and saw the crash on Korean news. I just kept thinking about the things the conversation with the guy on the plane.
Basically, these guys were incompetent through and through. I am a 777 pilot with over 8,000 hours on the aircraft. Still currently on it out of SFO. All a competent pilot needs on a clear sky day is his eyeballs, and airspeed. That is it. As soon as he realizes the glide slope was inoperative (after missing that fact on the ATIS), use Vertical Speed while on autopilot and put the green arc on the runway threshold on the navigation depiction. Better yet, just click the AP off and manually fly the yoke and throttles. Easy.
These Korean pilots do not know how to manually fly an aircraft without automation. Think about that for a moment. Absurd.
This is developing world pilot training and qualification requirements problem. OpSpecs of most outside US airlines have “must”use autopilot requirements down to 200’ AGL above touchdown and at 100”AGL on take off. I hate to say it, but Europe is like this as well. Also very rare to have military trained airline pilots outside the US. We (former military pilots) have all kinds of hands on manual flying experience and continue to hand fly aircraft below 5,000 ft most of the time.
These guys are an embarrassment to airline pilots in general, Koreans, and Asiana Airlines.
Wouldn't it make more sense to use FPA mode?
You're questioning an ex-military pilot with 8,000 hrs in the 777 lol

It's really surprising that there were "only" 3 fatalities given what this plane went through once it was close to the ground (one of them being from the SFFD accident). The video of the crash from an airport building where you can see the aircraft flipping around showed how violent the crash was, and even with all this tumbling only one person inside died.
In fact one of the fatalities was that of a Chinese student possibly run over by an attending fire truck .
@@richardshiggins704 Yes, that's who I was referring to by "the SFFD accident", SFFD is the San Francisco Fire Department. It was a mess. Even though there were some inconsistent reports initially, there is now no doubt that she was run over. SFFD described it as a "tragic accident" and said the passenger "was alive and lying outside the plane near one of its wings when the trucks ran over her". The parents sued, but somehow the case was settled without a payment. The firefighter who was driving the accident truck sued the city for being made a scapegoat instead of SFFD taking responsibility for the overall botched response, and he settled for $250k.
@@desmond-hawkins There's no doubt she was run over, however the question is whether or not she was alive prior to being run over. SF covered it up stating she was deceased already. However the city coroner said she was alive. An autopsy determined that she died from "multiple blunt injuries that are consistent with being run over by a motor vehicle. Our forensic examination revealed that she was still alive when she received those injuries". Also the case was settled with an undisclosed monetary payment. An attorney for Ye’s family, Gretchen Nelson, said the parties had reached “a confidential settlement on mutually agreeable terms,”
@@Passions Ah yes I remember these disagreements over whether she was still alive. I'm not where you're getting the "undisclosed monetary payment" though, where is that from? In an article about the lawsuit being dropped, which cites the same "mutually agreeable terms" quote as you did, they report: "The city attorney's office said no money was paid to the family to dismiss their lawsuit".
Well, may that "only one person died," rest in peace! My condolenses to his/her family. 😢❤❤❤

Pilots dont disturb passangers, passangers are typically excited to see pilots, theres no reason to keep the relief crew cramped in the cockpit.
I know there's a lot going in the cockpit during a descent, and they were probably fatigued, but I find it baffling that someone could move the power back to idle and then a few minutes later think "why is the power not increasing" but not make any attempt to increase the power.
"Shame culture" is a very powerful incentive to avoid getting to grips with any deadly errors involving oneself as well as the mistakes being made by one's peers. The decision to "Go Round" should have been priority ONE and a lesson in proper airmanship for all concerned.
He thought that auto throttles were engaged
They expected the automatic response of autopilot instead of actually flying the aircraft...
@@stephendavis6066even then, if he was looking on his speed, he would also see the "HOLD" sign on his flight computer and if he didn’t get responds from the autothrottle, he should have probably disconnected it or atleast put it into HOLD mode.
@@stephendavis6066 if the automatics aren't doing what's wanted of them and the correct response is to disconnect and get in a safe position to see what happened (in this case, go around) and then attempt the approach again. A go around doesn't hurt, not doing it does kill people

Let's just give a little hat tip to the NTSB employee who leaked these names in the early hours of the investigation in order to mock journalists. They were truly ahead of their time in terms of disdain for traditional media.
Not sure which is more outrageous; the fact that the news coverage of the crash was used to make racist name jokes, or the fact that one of the passengers thrown from the aircraft on impact survived, and was subsequently immediately killed by a rescue vehicle running her over.

I have worked avionics on the b-1b in my youth and have continued to work avionics to this day on helis. I'm nearing 50 and set to retire soon. As the world gets dumber and more things become automated for the masses. Expect more and more crashes like this in the air and in your cars. No one has the brains required any more to keep you safe. They all rely on the automation to do their jobs.

There's no excuse for 1 hour private pilot to let their speed decay on approach on a beautiful CAVOK day, much less 3 professional pilots.
The one excuse which has any validity is the company's de-training via excess use of automation and lack of recurrent training just flying the plane (now called hand-flying). Even so, airspeed (AOA to be exact) is life and you can't automate that fact away. No matter what else is on your mind, you keep a margin above stall speed at every altitude, in every config. Sure, we're all susceptible to body-clock issues, but airspeed is so basic that's it's drilled into student pilots from their first lesson and is (or should be) instinctive, and no company procedure should de-train that instinct.
They were not doing an autoland, they were below the altitude at which AP/AT would be normally off, and even at max automation you've got to scan your airspeed on an approach. It's a great plane (last Boeing designed by pre Stonecipher people), great conditions, they were going into a long runway. Perhaps company SOPs deserve some part of the blame, but no matter how bad your SOPs you must know that airspeed (well, AOA) is life and it's your job to keep that life above the flatline of stall.

You’re a bit sketchy with the descriptions of the AFDS modes. Even your graphics show HOLD / / VS, which isn’t a possible mode combination. It’s always SPD / / VS and the autothrottles are always engaged and active. HOLD only occurs with FLCH and VNAV PTH where the aircraft is,essentially, in a zero thrust glide. VNAV PTH wakes up the A/T’s for low speed to give SPD / / VNAV PTH, but FLCH doesn’t. It’s a zero thrust descent mode that uses pitch to maintain the selected indicated air speed.
The confusion around this is why the plane crashed, and you’re sharing that same mode confusion.
Your videos motivate me to study aviation
Glad to hear that
Truly, an extremely anxiety-inducing video. Seeing how thousands of hours of experience, with multiple pilots, and a perfectly sound aircraft -these pilots managed to never actually learn how to fly their airplane. This is sheer negligence, both from the pilots and the airline. So negligent in fact, that the pilots went full circle and almost became victims of their own ignorance. I cannot bring myself to feel bad for them or be empathetic towards their decisions, as even after crashing a fucking airplane, they didn't call to evacuate. Perhaps if this accident had happened on a privately owned aircraft, and the pilots had been new to flying and crashed, I would feel bad. As they would be the only ones affected by their mistakes (in a sense). But passengers are introduced to the equation, and a higher standard is placed on the pilots. A standard that they did not even come close to meeting.
Great video, great graphics and explanation. Bit of a downer tho lol. 😂
Daaaamn Sum Ting Wong :(
You make mention of the safety record of ASIANA. I’d question this with reference to an incident at Anchorage some (many?) years earlier involving a 747 which turned into the wrong gate. Rather than waiting for a tug to push them out, they elected to attempt a U-turn and in doing so a wing tip became lodged in the tail of an adjacent smaller parked aircraft. This mess was further compounded by the Asiana crew bringing the engines up to take off thrust in an attempt to dislodge, this resulted in baggage carts being propelled through windows in the terminal building. Asiana were banned from Anchorage for a number of years.
Fun Fact: My uncle was on vacation in San Francisco on this day. He had literally just arrived at SFO, and he witnessed the aftermath of what happened seconds earlier.

Correct me if I am wrong. I assume each airline has all the manuals for each acft type but do all flight crews receive the same training. In other words if the acft is a 737max do United crews get the same training as Southwest crews or Japan Airline crews, etc. Why don't all crews receive exactly the same training regardless of what company they fly for? If they don't , I don't like this at all.
Pilots are NOT being trained to fly 100% manually has cause way too many crashes and deaths. Automation is okay at cruse, but not for takeoff or landing. Things NEED to change.

Unbelievable .
Surreal. As a pilot, even _before_ all this automation, the most vital parts of your job have always been to get the plane into the air and then back at the ground. If you're not prepared to handle even a _partial_ loss of automation in otherwise perfect conditions... god, how do these people make their way into the cockpit?
The survivability of the crash was probably aided substantially by the low speed of the plane and the proximity to ground as opposed to water.
That NTSB intern lost his job for providing false names to the news. But honestly the news should have fact checked the data.
