You're not liable. 1. She didn't even know shebhad the allergy. 2. We have ingredients listed for all the soaps so if she did know, it's her responsibility to not use that product. However, to keep a customer, i would have given a partial refund in the form of store credit.

Not your fault. Refund her for the price of that one sample bar. She used the other sample bars without a problem, so she should not get refunded for those.
