I could be wrong, but if I recall, I think he said at some point that he wrote this after having a dream about a vision like that. And the train represents various addictions and ways that people escape and hide from their problems, rather than facing them head on and looking to God for the strength to work through them. Extremely powerful, and one of my favorites, too. I've seen the video dozens of times and I still tear up a little.

The song seems to be saying that Satan uses any means possible to keep us from following God, and that when we give into temptation, the consequences are alcoholism, depression, unwanted pregnancy, etc. But the song also says that God is willing to forgive us. "I stand at the door and knock." (Rev 3:20)
You got that right; when we’re tempted, we’re pushed away from God. Jesus knew straight away Satan was tempting Him when He fasted in the wilderness. But each & every time, Jesus said “No!” followed by Scripture to back it up.
