She did survive the childbirth...I remember when the song first came out, before the video, nobody knew if the girl lived after giving birth. That was shown in the video. I interpreted the first part of this song a little differently when I was younger. I thought the boy and girl were friends, which is why she had a fishing pole and wanted to go fishing with them. But the dad was telling his son we can't leave her behind, as in she can't stay at their house alone, they need to take her home. And when he said son I know you don't want her to go I thought that was like the son doesn't want the girl to go home. He wants her to go fishing with him. Someday you'll change your mind, I thought was the Dad's way of saying like you know someday as you get older and things change you'll grow apart, because that's just kind of a common thing that happens in life. Of course that can be difficult to explain to a young kid so it's just easier to say you know someday you'll change your mind. And then the next part singing about Jimmy Tommy and Bo, I thought was like send them home but don't take the girl, because he had a childhood crush on her and wanted her to stay with him. So then I thought like the rest of the song was kind of like in a way, proving his dad wrong that what might have seemed like a silly childhood crush didn't fall apart as they got older, it only got stronger.

The ending to me is sort of open ended. It never specifies whether his wife lives or not. The video shows a woman or possibly another girl as in the beginning of the video running to them at the end as well. I think this may have been his first huge hit. Although huge hit is his song If You're Reading This. Fair warning it is gut wrenching.
I disagree.. it's not open-ended at all... each verse starts with "don't take the girl" but ends with next verse continuing the story.. so there was never a doubt that she survived.. last line is repeating, "Johnny's daddy was taking him fishing"... So I pictured the baby boy named Johnny, its come full circle, with a continuation of family story... The point of the song is to show the lifetime of 2 kids and how love grows.. not sad at all.
"The Girl" at the end was the mom -- his Prayer was heard. Indian Outlaw came out shortly before this one.

Maybe the baby survived, and his wife didn't? The Dr in the song said the baby was doing OK, but his wife "was fading fast."
