Sir, I lodged my visa 491 with ICT trade occupation on May 2022. My application is complete with PCC and Form 80 and 1221. A CO asked me to re-do medical on August 2023 which I completed. I'm still waiting for the grant. When do you think my visa would be granted?

I have lodged my application for 491 on 19/06/2023, all pcc and hospital check all is done, but still CO is not appointed, my trade is fashion designer - 232311, so what will be the visa grant time?
It’s generally from 12 to 16 months so you still needs to wait for another few more months
My Immiaccount last update change to 16 jan from medical date but nothing else. What does mean ?
You just need to wait for further assessment. If case officer requires any further documentation, they will get back to you

You can check the processing time of your application from the Department of Home Affairs' Global Processing Time.

Your occupation is not on the priority list so it will take another 2 more months to allocate to the case officer. Nothing to worry about. Just make sure you have submitted all documents as per the requirements. If you have an agent, ask the agent to double check everything.

Even my agent called DHA in June they said CO had assigned to your case but my status is still received.
Even Co told them to refrain from DHA and stop interfering.
From that day , still my status is received
We already have enough librarians in Australia, we don’t need you as your skill set doesn’t benefit us. Try find an occupation that will actually benefit us, otherwise why would we want you in our country.

You have to wait for until your visa gets approved then only you can apply for your parter 491 dependent application.

Once a 491 visa is granted for offshore applicant. How long will they give to enter austraila ?
Generally its with in 6 months to 12 months time. You will find "Must not arrive after" or "initial entry date" on your grant letter.

Skilled visa applications are processed in the following order of priority:
1. Visa applications in relation to a healthcare or teaching occupation.
2. For employer sponsored visas, visa applications where the applicant is nominated by an Approved sponsor with Accredited Status.
3. Visa applications in relation to an occupation to be carried out in a designated regional area.
4. For permanent and provisional visa subclasses, visa applications that count towards the migration program, excluding the Subclass 188 (Business Innovation and Investment (Provisional)) visa.
5. All other visa applications.
For all categories above, priority will be given to holders of eligible passports.
Within each category above, for provisional and permanent skilled visa applications, priority is given to visa applications where the primary applicant is located outside Australia at the time the visa application is made.
The Department will contact you if additional information is required.
Your correspondence will be sent to the relevant processing area for information.

Your points are on lower side. You would be needing around 75 points for 190 whereas 491 is on the fine side. Generally rounds are too slow these days so you might need to wait for 5 to 6 more months.

But I concerned about pte score, will I get PR with proficient english.
The invitation will depend on the state you've applied for. if you're going for VIC, they are mainly focusing on salary and higher points. However, it is advised to go with superior English for better chances as compared to proficient English. Hi Julie, please note if you are offshore than unfortunately Recruitment Consultant is not in QSOL list. This means you will not be invited until the occupation comes to the list.
