While I read the instructions it says each individual has to sign their own copy of this form. What I can make out is either I make as many copies of this form and get it signed by all of the principal applicants children.

Hello, thank you for the info on what to fill in the marital status for the deceased. I have been trying to find out what to put for the deceased. I thought of putting unknown and then attached a letter but I will go with your advice.
Please when you click on validate does it show barcode? I clicked on validate and it shows no barcode or says validated.

This form does not generate bar codes. Write the status at the time of death and explain this in your letter of explanation. This was done by someone and was successful
