If the filter is 18 years old & let's say it's clogged up, would it make the system moan really bad?

Yes, that is very well possible. Keep in mind if you change it. It will not hurt.

@@dairleautoclassicrestorati6892 thanks for your reply. We changed it & the noise was still there. But we figured it out, it's the big bearings behind the fan/clutch that have died. Just happy to have found out the culprit

It would not hurt to change the Filter. But most likely it's your Power Steering Gearbox.

I don't have the international part number off hand. But you can use any Power Steering Fluid just as long as it's not Transmission Fluid.
