If this is editable pdf form, then open in pdf and fill all the details. Then take a print and sign. If this is not editable pdf and take a print and fill up. It’s not required to fill up in capital unless it’s mentioned for specific fields. Thanks

This form is for biometric. There is another one for passport submission if your query is regarding submission of passport. Unless you are planning to stay long at US address, I suggest to use the passport address in all forms. Thanks

Hey! Biometrics once given are valid for 10 years. So, since your mom submitted biometrics for a visitor visa she was not required to submit it for the supervise. However, even I am unsure about which section to fill in the biometric section though on the consent form.
Once you submit biometrics, this info would be sent to IRCC. It depends on individual file how it progresses. You should wait for at least 2 weeks and then contact IRCC for it.

Is it okay to fill Aadhar Card address in the consent form, as I am not currently at the address on my passport and don't have any proof to verify. The passport was applied for during college time in another state. Please suggest.
My suggestion it to first change passport address to current one, then go for any visa work. You never know when they could be any communication or verification on that address. For any Embassy, they just know address on passport and not any other document.

No, that section is not for minors. Also, I think for minors you will not even receive a biometric request so this form shouldn’t be required.

If your VFS office is not asking then no need. But most of the cases (almost all), they would ask for it.

@@timiracheal430 first check your ircc account and it will have the msgs with letters. If not, you should talk to vfs helpline and they will help.

You need to have letter for an appointment. Contact VFS & IRCC asking this info. My advice, dont delay beyond the time period mentioned in your BIL.
