Harley Quinn was a therapist, not a nurse. It looks to me like the musical elements will be only in their shared delusions. If you notice, Harley is outside of the bars and on the outside of the prison partition. I'm thinking she's not a patient
thank you for the comment! Did I say that she was a nurse? I knew that she was his therapist, so I may have just said the wrong thing. I think she is a patient though because the first time we see here she is dressed in shabby clothes and standing in a room full of people that also look like patients that are doing what looks like some kind of musical group therapy (singing together from the papers they are holding).
