Androgel is a commercially made transdermal testosterone preparation in what we call a carbomer gel, which uses alcohol (about 67%) as the delivery vehicle. Alcohol generally does not sit well on tender thin skin like that of the scrotum. It can burn, especially if it gets near the anus. This is why they have you avoid those areas. Additionally, because of it's relatively weak concentration of T, you have to apply 5-10 or more grams of gel to get therapeutic blood levels. That's kind of a lot to smear all over your boys, so they direct you to larger surface areas (no judgment whatsoever there with the adjective). Compounding pharmacies generally concentrate the testosterone in their formulations requiring less volume to deliver the same payload, and use bases compatible with sensitive skin. The scrotum is ideal because it is out of the way (avoids contact transference to others) and its very absorptive. We get good levels when applying there. And... as you mentioned. Compounded testosterone is MUCH cheaper. Compounded testosterone is often covered by insurance. Ask your compounding pharmacy. Thanks for your question, Mr. Chip.

Hey mr k. Do you feel one application of a 10pct cream will give clinical ranges. So instead of 2 clicks for 100mg it would be 1 click for 50mg once a day. Your thoughts?
I am 62 with high PSA 3.7, my Dr wants to start me out with 200 injection once a month. He says insurance does not cover cream, though my insurance said they do
I presume by "eod" and "ed" you mean every other day and every day? I like that dosing schedule, AND it can be dosed sub-q with a shorter needle, so less painful. You use the same weekly dose (commonly around 1ml of the 200mg/ml testosterone cypionate) divided by the number of doses you give in a week. Be sure and get 3/10cc insulin syringes with 29 guage 1/2" needles. If you try and use the short needles, they are smaller around (31 guage, which will be like sucking honey through a straw) and won't work well. I have patients who do daily SQ injections. It just requires some patience pulling the oil up into the smaller syringes. You should check with your doctor if you want to administer your testosterone differently than the label directs. My husband had a baseline testosterone of 100ng/dL. His doctor suggested testosterone injections. Would the cream alone get him in the normal range or with someone that low of numbers benefit more from injections?
Testosterone creams have always gotten a bad rap because "bro science" from the meatheads at the gym will tell you to stop being a wuss and take the needle. BUT, the truth is, all recent studies have shown that the injections and creams have the same effect in raising testosterone levels.
Hi Fred. The body naturally pulses the release of sex hormones so that receptors do not develop resistance. So morning OR evening is okay. It's best to make that decision based on when you are less likely to transfer your topically applied testosterone to your partner. The decision to apply hormones to the skin should be made by the owner of the skin! Now, having said that, testosterone drives libido and more so at higher levels, so if you want to apply it closer to the time of sexual activity, you'll probably like that. But give yourself 2-3 hours after applying it before rubbing that skin on somebody else's.
Thank you. And, in case you're wondering, your boys are not going to shrink away if you apply testosterone to your scrotum. You can expect perhaps a 10-15% reduction in testicle size with ongoing testosterone supplementation simply because you are relieving your boys of some of the work of production, but you likely won't even notice. It's more with age.
Hello,a question: I have been using compounded testosterone cream 200mg/g 50ml 100mg testosterone per pump for three months now, normally i had to use 100mg 2 times a day, so 200mg in total, but I only use 50mg 2 times a day (AM/PM) , after three months i did a blood test to the 12 hours before the second administration and my total T was 350 and free T 5.62 so especially free T is dramatically low(( so my question is, is it normal that my Testosterone in the blood has dropped so much after 12 hours or is it just my dosage is too low? Thank you very much in advance!🙏
Any TRT clinic worth a damn won’t prescribe injections every other week 🤣. I like your content but dude, that’s got some serious spin to it. I could make a video saying “injections once a week vs creams every 3 days” and completely flip the narrative. They both have efficacy for sure. But doesn’t an IM injection allow the body to more effectively absorb it?
Hi, I have heard topical testosterone raises DHT levels significantly. Is this true, and should I be concerned of balding? Also thank you for the video
Hi! You're welcome. It is true that the topical testosterone route raises DHT levels higher than injectable, especially when applied in the genital area. This is because there is a higher 5 alpha reductase enzyme level in the skin and genitals, which is what converts testosterone to DHT, but here is the hard truth. The likelihood of male baldness is most influenced by genetics. And those predisposed to it genetically can experience hair loss from even normal circulating levels of testosterone, so I think DHT concentrations are a bit of a wild goose chase. Yes, they bind with a higher affinity to androgen receptors than testosterone, but your receptors are likely already saturated, so we're back to genes. Thank your parents. I really don't like blocking DHT (with finasteride or dutasteride) because it's a party killer. DHT is a much more potent stimulator of libido that testosterone and you don't want to be without it. Besides, haven't you heard? Bald is sexy, man! I know, it's mostly the bald guys saying that.
Cream?? Not a chance. If the levels are to high and to low in 2 weeks administration do it every week cipionate. Cream you have to do it every day, you cant sweat after, you have to wait for the absorption. Not for me.
I don't mind your take on the subject. You just simply state the cream is "not for me". I see so many people saying they won't use cream because it's not as effective as the injections. Injections have been the gold standard for so long that people are afraid to try anything new. Yes, you do have to administer every day but in reality, it's not too bad, apply, let it dry for 30 seconds, go on your way. You just can't hit the gym or have sex for 5 hours to avoid transference. The reality of it is that all recent studies have found the creams to be just as effective as the injections so it's a matter of personal preference. It's a personal choice and you like what you like.
No, its just that people vary and some skin is thicker/thinner than others. For X percentage of men, topical just doesn't work. In this case, its now a question of convenience, rather, one of necessity. After all, who would prefer something you rub on? Not to mention is a whole lot easier to travel with.
I use the cream twice a day. I used to do TRT one shot a week and I will NEVER go back!!! I feel so much better under my current TRT protocol!
Very interesting feedback! Glad to hear that you've found a protocol that works for you! Thanks for sharing.

Thank you good report. Where can I get this compound testosterone with my doctors prescription of course? What is the brand? Thank you. Finally a video for men. Straight dope on how things work. Thank you.
You can get testosterone cream at a compounding pharmacy. Chances are there's one close to wherever you live. You could get it from us (Mt Olympus Compounding) if you happen to live in Utah, Wyoming, or Nevada. Those are the only states we're currently licensed in. If you google "compounding pharmacy near me" you should be able to find something close. Let me know if you have any troubles. Thanks for the comment. I appreciate the support.
1:13 this is kind of comparing apples and oranges, if you were compare this with microdosing testosterone injecting every day in the morning,
you would have the same effect.
No endocrinologist in 2023 is doing this old brutal injecting once every week or 2 anymore.
Hey mr k. Do you feel one application of a 10pct cream will give clinical ranges. So instead of 2 clicks for 100mg it would be 1 click for 50mg once a day. Your thoughts?
I am 62 with high PSA 3.7, my Dr wants to start me out with 200 injection once a month. He says insurance does not cover cream, though my insurance said they do
When doctors insist medication is only available one way they're usually playing salesman for big pharma.
That's a joke of a protocol.
3 injections per week with the dose split… or daily micro injections give way more consistent levels than daily cream.
Very informative
What about eod or ed injections
I'm just giving you options to discuss.

Straight to the point and one of the most informative videos on testosterone delivery systems I’ve ever seen. Ty!!
Actually it is not. You need to educate yourself.
@@jimpurewal2661 I am trying to educate myself and it’s an uphill task when it comes to HRT for women. Perhaps you’d like to help with information instead of condemnation.
I’m just starting out here in Australia & they have given me 1ml daily (10%test & 1.5%DHEA) & have been told not to put it on scrotum ?? Why only once a day also ? I feel like this won’t work as well as applying twice
The gel is better for me. My hematocrit and RBC' don't move with gel.
Im moving to NC. Could you recommend a dr. That would prescribe this for my low t? I dont want androgel and the clinics just push injections and charge way too much. Thanks!
Hey Trevor. So sorry for the delay in responding. Please contact me by email and I'll hook you up. layne@mtolympusrx.com
@@hormonespecialist Hello do you know of a good doctor near Winston Salem NC? I just moved here and I need to find one I’ve been treated for low t since I was 28 . I’m now in my 40s
I have a question I really hope you could answer.. cream testosterone might not is what I need
When I was a baby according to my neurologist I had a brain stroke in the right side of my brain, now I’m 36 my left leg is much smaller than the right one like the calf specially..can something like cream testosterone help me to get the muscles bigger ?
Is Androgel (which my insurance is covering) considered a creme ?
It is considered a topical testosterone. So for the purposes of our discussion, yes. But it is technically a gel.
Years old!! It’s subcutaneous now, daily or E2D microdosing.
My husband had a baseline testosterone of 100ng/dL. His doctor suggested testosterone injections. Would the cream alone get him in the normal range or with someone that low of numbers benefit more from injections?
Gel is better.. injectable is even better..both are better than peaches or pills..to feel happiness in all he should be over 800dl/ng
@@commantrosexetlos4700 Incorrect. What better is what words and the fact is, the gel is the least reliable of all the administrations.
No way to tell. You have to try it and see what happens. How he feels and what his blood work shows.
Testosterone creams have always gotten a bad rap because "bro science" from the meatheads at the gym will tell you to stop being a wuss and take the needle. BUT, the truth is, all recent studies have shown that the injections and creams have the same effect in raising testosterone levels.
as a RPh, is it better to apply the cream in the morning or the evening to simulate the body's natural release?
I think the half life of he cremes is only 12 hours so you will probably want to apply twice per day.
Excellent video. The spikes is why I chose transcrotal cream over injecting.
Hey buddy what brand do you use
Once a day or twice a day? How do you feel?

Why talking about injections every 2 weeks when most men inject for trt once or twice per week?
Exactly.
I mentioned it because I occasionally see it prescribed that way, but you're right. That's way too seldom.
Good point
Here is a new video we made about testosterone, DHT, and baldness in men.

$ 700/month for Androgel?? More like $50-60.
Back then vs not I suppose?
Once a day or twice a day? For the cream.
Once in the morning right when you wake up
It's a personal choice and you like what you like.
real men use needles
No, its just that people vary and some skin is thicker/thinner than others. For X percentage of men, topical just doesn't work. In this case, its now a question of convenience, rather, one of necessity. After all, who would prefer something you rub on? Not to mention is a whole lot easier to travel with.
Drink some water. Geez
