It is very clear that Karamo disapproves of the relationship because of the level of toxicity. In this instance an apology is not necessary because the host and most of the studio audience believe they should end the relationship and move on. No one even clapped at the proposal because they've noticed that their relationship is unhealthy.
I believe Karamo would have asked her to apologize if he thought the relationship could be and should be mended. I can't fault him on this.

Although they should be separated, Karamo still chooses who he lets off the hook without any accountability, and who gets the accountability at times. This was just another one of those episodes. I’m not saying he needs to be perfect because no one is, but Karamo has a noticeable pattern of calling out only one individual and not the other and just laughing/joking about it with that particular person. Usually, he doesn’t stand for any type of ignorant behavior on someone’s behalf (not saying he has to stand up for it all the time), but he’ll call it out. But recently, he lets some people off too easily, even WHEN they are continuing a relationship. You don’t have to agree.
