Back Pain and Sciatica Workshop Explaining the McKenzie Method

  • In this workshop on Low Back Pain and Sciatica we discuss how we evaluate patients with lower back pain and sciatica using the McKenzie method based on internationally known physical therapist Robin McKenzie. Robin McKenzie developed this approach is over the course of many years of refining the system of evaluating the lumbar spine and cervical spine. His approach is a systematic way of moving a patient’s spine to see if it has a position or movement that reduces or eliminates the patient’s current sciatica or lower back pain. During the discussion we also discuss the red flags when experiencing lower back pain and sciatica. We discussed when the patient experiences progressive weakness, severe night pain, balance problems or loss of bowel and bladder were experiences of trauma or fall seeing a physical therapist should not be the first person to see. We discussed some common back pain myths in our workshop. Some of these include the belief that you need an MRI were x-ray before starting physical therapy or conservative care. We also discussed the high probability of having a herniated disc bulge even if you don’t have any history of lower back pain and sciatica. Also, we note studies that show that when an MRI is done too early the results often make the patient more apprehensive and nervous about engaging in exercise. We also discussed risk factors for lower back pain and sciatica which include; obesity, smoking, previous history of lower back pain, static postures, and a family history of lower back pain and sciatica. We provide our top three exercises that I provide patients in my clinic for lower back pain and sciatica and the precautions when doing these exercises. We also discussed the concept of what is good pain and bad pain using the ideas of centralization and personalization. We demonstrate how to use a lumbar roll for sitting and driving and also proper sleep positions that are recommended for prevention and during episodes of lower back pain and sciatica. During this video we discussed the adverse aspects of greater than three days of bed rest in trying to alleviate lower back pain. The research shows greater than three days of bed rest can cause greater pain and disability. My general recommendation is to move as tolerated, not sit more than 30 minutes at a time and perform the one exercise we found to alleviate the symptoms of lower back pain during the McKenzie evaluation. At Apex orthopedic rehabilitation we utilize the McKenzie method and evaluation techniques. The McKenzie lumbar evaluation is a systematic evaluation using patient generated forces and later therapists applied forces depending on the patient. The general recommendation is to perform these exercises two times every two hours over the course of the day. If in 2 to 3 days the patient does not have a significant change in his or her symptoms were condition the exercise may not be appropriate where the patient may not be preforming it correctly. Also, the patient may not be a candidate for the McKenzie method or needs to be progressed to the recovery phase that comprises of a general strengthening and flexibility and endurance program.
