I rescheduled and booked OFC: 4 Oct & Consular: 7 Oct.
I confirmed the new appointment, but the old OFC date (25 Sept) still appears on the my appointment confirmation. Which date i should go to OFC

Since its 221g, you need to pay sevis fee and send receipt via vac/ofc
