In 3 phase 440v supply will not have any unbalance load.but in 220v supply distrubution is have as you said. so monitering 440/220v transformer temperature is important. Appriciate your comment.
In high voltage ships (greater than 1000 voltage) they are using earthed neutral system Source and load both are earthed with hull also source will be connected with NER "neutral earthed resistance" And connected with hull In high voltage ships earthed neutral system even at single earth fault black out will occur it will not wait till for double earth fault like isolated neutral system

The fault current flows in alternate path which having low resistance so the short circuit current level will shoots high than the normal level. In this case if any humans touches that short circuit equipment it leads to death also this short circuit current damages the source that's why both source and load are earthed to the hull
