    The 14 day PRN psych med thing is happening here in Connecticut SNFs too. Very frustrating for hospice patients. Love your videos! Keep em coming.

    Colorado paramedic here I wanted to thank you for this great video, you all have the biggest hearts on our planet. p.s. I never given Opiates to help a patient breathe because in my experience those meds cause respiratory problems but we do 2 different jobs. have a great day

    Thankyou for the information …my poor dad is dying but he is very peaceful and not in pain …it’s so sad but we are happy he is comfortable and not in pain xx

    Yes the 14 days is a CMS protocol and ties into the SNFs quality measures.

    I was traumatized when I was 14, I went to the hospital to see my uncle (this was in 1981) he had end stage pancreatic cancer and had, had open surgery 6 months prior to debulk the cancer so he had been on strong pain medication. That day when we got on the floor my Uncle James was screaming, I wasn’t allowed in the room, but I heard a nurse tell my grandmother that they had given him all the pain and anxiety meds that they could but he had built up tolerances so there was nothing else they could do. Is it better now? What do you do when you have a patient that’s been on high opioids? How do you give them comfort?

    Roxanol 20mg/1ml to give 5mg it is 0.25ml not 2.5ml that would be 30mg. Alot of times it has to be increased to 0.5ml=10mg then 0.75ml=15mg or 1ml=20mg q 4 hr prn or sooner if pain is not managed for 4 hours. 18 years hospice RN. Great video!

      My grandmother was on that and 2 mg Ativan and I think Haldol when she was in hospice. She was definitely relaxed.

    Without the morphine, how long can a hospice patient stay on the other “3” drugs? Ativan, Haldol and and psychotropic drug

    My dad almost died due to hospice care. They were given him Morphine/Ativan which turned him into a zombie and he wasn't able to eat/drink... Stuff is horrible

      So sorry you had to go through that.. Unfortunately it seems to be far too common. Hospice should be about 'quality of life' and making the most of the life that remains, not causing premature, unnecessary death. This 'morphine cocktail' is chemical sedation, and should be avoided, as these drugs given together SHUT DOWN respirations. Pain can be relieved safely without danger of death, when Morphine is given in the correct dose by itself. I have been in healthcare for over 35 years and have never needed to give a Hospice patient Morphine AND Lorazepam together to achieve comfort.
      Hospice does a good job of teaching family that a patient 'needs' this, and all too often family will then expect the patient is going to receive this 'death cocktail' on a scheduled basis.. Thinking they are doing the right thing for their loved one, when they can be ushering in death prematurely and unnecessarily. Family needs to pay close attention to the signs their loved one is giving, and if they are even showing signs of pain to warrant Morphine being given every two hours. Death is a natural phase of life, and doesn't automatically equate with excruciating pain.. Some of the most severe pain a person will have is after surgery, post op, and we would never give Morphine and Lorazepam together for that, but yet find it 'appropriate' to give a Hospice patient. These two drugs are contraindicated to be given together, as they potentiate the side effect of respiratory depression, yet Hospice will tell you that they are 'safe' to give together. There really is no reason to give these two drugs together. Giving the correct dose of Morphine or Ativan should be effective to treat whatever symptom the patient is displaying.. Pain vs anxiety.. 'Doubling up' leads to over medicating.

      @@Ohmygosh60 so horrible! Get them off that stuff! Put it in writing! No morphine/half ok/Ativan etc... write reviews online too! It's murder what they get away with. Sorry :(

      I always say to wait 5-10 min before drinking so the medication can dissolve effectively:)

    they only prescribed my dad .25ml of morphine, every 4 hours, which doesnt seem to do much of anything

      No you do not. If they are an alert patient they may complain about the taste and it’s fine for them to have water if they can tolerate it.
