PACE CNY Provides an Alternative to Nursing Home Care

PACE CNY is a 'Program of All-Inclusive Care for the Elderly' offering a complete range of healthcare services for seniors 55 and older who are eligible for Medicare and Medicaid in Central New York.
The goal of PACE CNY is to help older individuals live in their homes and communities for as long as possible by providing the services they need, such as medical care, home care, transportation, and prescription management.
Each PACE CNY participant's care is managed by an Interdisciplinary Team of doctors, nurse practitioners, social workers, rehabilitation therapists, dietitians, and recreational therapists who provide for their medical, rehabilitative, social, and personal care needs.
