Actually, the bad ending in Mafia III isn't any harder to get than any of the others. Regardless of how many of your lieutenants are left at the end, if you choose to go to the mansion at the end, you'll get the choice to 'Rule Together' or 'Rule Alone'.

I absolutely hated the Rule Alone ending from Mafia III. I’ve watched all of the endings, and it’s my least favorite. Spending the whole game fighting gangsters and avenging your family, only to end up being worse than the people you just killed.

My dad and I used to play the game together a lot, and when I discovered there was a bad ending (I think because of the sound/music menu), we tried everything we could think of to get the bad ending. Except, apparently, telling the big bad no (or we could just never beat him because of the PvP thing). So thank you!
both the dark souls 1 and 3 age of dark endings are the good ending. the world is in the sorry state its in, because the gods refuse to allow the age of fire to end, for fear of losing their rule. the age of dark allows natural progression to finally continue and in time usher in the age of humanity, where gods are no longer needed, because mortals can produce their own flames. the endings seem bleak, but they break the cycle and give hope.
Elsinore has such an ending. You basically have to kill everyone around, and since most NPCs are in the same area as others, you have to be careful to avoid being caught.
Streets of Rage has another caveat. If you say you want to join the bad guys and defeat your partner, if you say yes join the bad guys again you're kicked out of the building and back to the street. You have to do the last few levels again then fight the big boss.
A very hard bad ending that a few people know is in the fuga melodiees of steel series, in both games you need to sacrifice all of your characters to get the worst ending and that makes the whole game way harder
There's something you forgot to mention about the Genocide Route of Undertale (though It's been so long infamous that there's likely very few who don't know, spoilers ahead to those that this applies) . . . . . . . . ... and that's how you end up actually becoming a monster when you lose your soul to the true big bad, who's been dwelling inside you all along while apparently influencing your notorious behavior. Not only is this quite the bleak ending, it also affects all your future playthroughs. So unless you completely wipe out your save data (easier on some systems than on others) even your best endings will turn into bad ones, making your first Genocide Route truly the worst of all endings only reserved for the most masochistic of players.
I'd argue the DS3 secret ending is the best outcome since you end the cycle.
at the cost of wiping out all life, i'd hardly call that a good ending. what's the point in ending the cycle if there's no one there to appreciate it.

About silent hill, instead of SH3, I hope Silent Hill 1 will be the next remake. with different endings to finish it.
