Yes. They don't just go straight up to orbital altitude. They start with an eliptical orbit and then circularize it. That's why it takes 45 minutes.
Oh no! A maximum 9 minutes is sufficient, just after stopping the main engines and releasing the external tank. (Minimal orbit= ATO)
