    I know that there has been few explanations already for this, but there is still the assumption that people outside the UK understand all the references, so I'll try to give a more detailed explanation here for those who might need it:
    ER:
    The Queen uses ER as her initials, Elizabeth Regina (Regina is from the Latin word for queen), so it's like writing Elizabeth, Queen.
    ER (with the number 2 in Roman numerals between the two letters - because she is Queen Elizabeth the 2nd) can be seen on many government owned, or operated, institutions in England. For example the post office. All the mailboxes (where you put letters you want to mail/post), postage stamps, police helmets, some government buildings, etc., even on royal garments/clothing.
    The Queen will often sign her name as ER, Elizabeth R, or Elizabeth Regina.
    So, EiiR is a monogram used officially in England, and found on many things to do with the royal family.
    Birmingham use of "ER":
    Saying the letters ER quickly, sounds similar to how many in northern England would say, "Here you are" (or There you are), when giving something to somebody, or offering them something. It is the way English people say, "Take this. This is for you. Have this. I'm giving this to you."
    In northern England, "Here you are." contracted to - 'ere y'are. When said very quickly, it could sound like saying ER very quickly.
    So, the joke that Frank was saying from Birmingham, was, "Why does the Queen have so many children?" "Because she has the official ER written on her underwear. (sounding like local accent for 'ere y'are, "This is for you. Take this."), to be taken that she is offering herself sexually.
    It's just a silly joke, playing on the similarities of sayin ER and 'ere y'are.
    What is funny to the Brits (English people) is the way that Richard and David took a silly joke, and dissected the joke. Each one escalating the absurdity of the joke by adding to what the other just said. This is classic British humour, using wit and sarcasm together.
    Maurice Moss: You might see that reference in some of the comments.
    Richard played a character, Maurice Moss, in a show called The IT Crowd. His character displayed some characteristics similar to autism, and he would often take things said very literally, and pull them apart to show how ridiculous they sound. Many commenters here see similarities to how Richard dissected the joke here, to how his character Moss would react to something in The IT Crowd.

      I'm brand new to the UK so that was extremely helpful. I still need to convert my understanding of American English to British English for other parts of it but you explanation was still a big leap for me.

    David Mitchell is the best learned pedant in comedy since Stephen Fry was at his height 30 years ago. I've always thought Richard Ayoade was on the spectrum, just not as far along it as his character Moss. Together, they can demolish any joke to rubble.

      ⁠​⁠​⁠​⁠​⁠​⁠​⁠​⁠​⁠​⁠​⁠@@shamyself123sure, he’s probably playing it up for the camera, but characters aren’t invented from nothing. performance is like an extension of the self. i’ve noticed watching WILTY when david says something like “i have a collection of pens and a 3 rule pen-lending policy” that he comes across a little bit strange. he’s socially awkward, tends to take things quite literally and challenges social norms freely (he’s famously resistant to peer pressure and loves to rant about things he finds ridiculous) but spins it in a comedic way. not to mention his deep knowledge of history. of course, all of that can be true and he might still not be autistic - not that it even matters if he is - my point is that his persona just gives off those vibes. and a lot of the things i mentioned also apply to his character in peep show who i heavily HC as autistic. but that’s just my opinion 🤷‍♀️
      (i didn’t comment on richard bc i’m not as familiar with his work, so i can’t speak on him specifically)
      and yes, i recognize that this is a deeply autistic response lmao

    David Mitchell and Richard Ayoade don't get enough credit. I love them both so much
    Edited: I live in the US. They are not household names here. I have to actively go out of my way to find programming with them. Ask 100 people on the street here who David Mitchell or Richard Ayoade is, you might find one person who knows of one of them, probably Ayoade and only because Netflix finally added The IT Crowd. Chill TF out people.
