Don't worry about your accent. Just keep those big smiles and lighthearted small talk! No one understands anybody any way! Most American's can not pick out accents in general, as you know from watching UA-cam videos on the wild accents here.
My mother was in the hospital for 2 weeks before she died (no surgery) the bill was $43,000. Her insurance paid for it because my father retired form the US miltary. I was thankful for that!
Fun accent facts: Notice that the English dude does not pronounce R after vowel sounds. That is typical of most English accents. The Irish girl pronounces R after vowel sounds like most Americans do. But Irish people pronounce it much more harshly.
And the Irish don’t seem to have the th sound, so breath becomes, bret. That may not be the best example, but you’ll be able to pick it up if you pay attention.
Most people get health insurance, either through their employer, privately or with help from their state and federal programs. You pay a sum usually monthly, or a small portion each paycheck, then if you need medical attention most of the bill is covered. It can be complicated. I can't imagine how he had a 6 figure medical bill.
If you were involved in an extreme emergency situation such as an extreme motor vehicle accident or an emergency appendectomy, that later evolved into a lengthy inpatient hospital stay, it is definitely possible. This was probably the total of all charges before any insurance payments or reductions were taken from the bill.
I encounter different accents daily, and I truly barely pay attention to any differences at this point. It is extremely common. There's the rural areas where you may not hear many accents, but even in those areas you will hear Spanish quite often. Your accents wouldn't stand out too much in the majority of the USA.
Not paying medical bills wont jave you thrown in jail or anything. Itll just ruin your credit rating, and thats only if youve paid nothing. If you send the hospital 5 dollars a month for the rest of your life it wont much hurt your credit. Not that having a bad cresit score is nothing, but tou can still get by paying upfront.

I had to come in and comment when he said something about healthcare in America. Although the costs are very high, America has the best healthcare by far and even if you can't pay your bill, nobody in America can be refused any kind of emergency room healthcare. Most people have insurance or it is provided for them by their employer. Still many cannot afford it and that is where Medicare and Medicaid come in. For the poorest of America the Medicaid program provides good healthcare free. So when i hear people saying most Americans cannot get care i roll my eyes. There is another choice called 'Obamacare' was nearly made into law, but when people learned that the co-pays were absolutely criminal and often higher than the total cost of your bill, it didn't go over too well and now only the wealthy choose it and even they only use it due to their blind faith in there political hero's. People come from all over the planet to get the best healthcare in the world from America so that should tell you something. The main take-away should be if you cannot afford to pay the high costs, then you will qualify for Medicare or Medicaid and cannot be refused service if you need help.
Americans love cultural diversity. I think you'd find that people would be asking you "where are you from?" constantly, favorably regarding your accent. You'd feel very welcomed here.
Also a lot of Americans like brown eyes. A lot of them have said they like my eyes and they’re normal brown. (I’m an immigrant) I pretty much lost my accent but some deep Americans can still pick it up.

The lame truth is that a lot of British guys in the US will exagerate their accent to try to impress women, seem exotic and try to make themselves seen more intelligent.
Personally, I think that the way people see pale skin today stems from how they saw it for centuries: back when illnesses ran rampant because penicillin & vaccines didn't exist yet, people with pale skin were seen as sickly & at some point the term "deathly pale" came into being. I could be wrong, but like I said, that's my opinion. Many people feel that they're taking too long to tan naturally in the sun or they're worried about skin damage, so they opt for a spray tan (I would never do that because yeah, you could end up more orange than bronzed). I for one love brown eyes; they're very warm & inviting, in my opinion (some shades make people feel they could "drown" in them in a good way, i.e. they're just so lovely & warm you don't want to stop looking into them). And having so many different cultures in this one country, we love different accents (I myself like your accent, Irish, German, Russian - believe it or not, & Brazilian/Portuguese).

Technically fast food is more expensive. You mentioned that guy making his own Big Mac… for example we can get a pound of the good hamburger meat for $6.50 per pound 85% lean. It’s a premium brand too, and if it’s on sale you can get it for as low as $4.00 a pound. We usually make 4 quarter pound burgers out of that, you include the buns, all the fixings, even making your own sauce, all those items are more than one serving worth. Now I tried to look up how much a Big Mac costs, and it depends where you live… it varies from $4.80 to $7.50… but because it’s made for you as a service, there will be tax. Buying your own ingredients from a grocery store here in Minnesota, there is no tax on grocery food. And you can make up to 4 burgers. Obviously it gets complicated, and I’m terrible at math, but I think in the end even buying premium meat, it still ends up cheaper, just less convenient, but tastes so much better in my opinion 😉
