Handed to Niccolo Polo by Altair Ibn-La'Ahad, then handed to his son Marco. Eventually finding it's way to the Auditore lineage... The Codex pages were safeguarded by many until under pressure from advancing Templars Domenico Auditore scattered them to the Italian wind. It would be Giovanni then his son Ezio that would reunite all 30 pages to tell a story. This is that story, The Story of Altair's Codex.

Quick correction of an extremely common misconception, Leonardo did NOT edit the hidden blade to avoid the ring finger sacrifice, Altiär had a second hidden blade in revelations and kept the ring finger on that hand, as well as Bayek also keeping his finger untill he activated the blade with a clutched hand.
The ring finger sacrifice is a dropped tradition rather than a design choice, this too being explainable by Altäir remaking the hidden blade to not need the sacrifice as it was originally built
Yup, Bayek's ring finger was cut off by accident, and as others were joining Bayek as the original founders of the Creed, they voluntarily cut off their ring fingers. Even though removing the ring finger wasn't necessary, it still became tradition because the Hidden Ones made it as a mark of their commitment, and that commitment later bled into the Creed for future generations. However because assassins became easily spotted by their missing ring fingers, Altair dissolved that tradition, and soon later the Italian Assassins decided to brand the ring finger with hot iron instead of cutting it off, resulting in a less barbaric, but more humane method of showing one's commitment. As for other commitments to the Creed, several Creeds decided upon other marks, such as Shao Jun wearing a jade amulet with the assassin's crest, and others that i can't remember.

It surprises me that Altaïr living in the 12th century is way more intelligent than some characters from AC3.
I told my wife she was drawing her eyebrows too high, she looked surprised.
The original use of the Hidden blade was a punching dagger that would activate when you pull your hand downwards in a Fist shape that would cause the blades wire tricker to pull it out. How This aspect of the proper use of the blade was never followed on with . Because the next time that it was shown used within the games was by The magi assassin He had to put it on underneath his wrist instead of on his arm so that he could Hide it when he went into the Baths but when he made the Motion to bring the knife out he had to put his fist downwards in a punch which forced the knife threw his ring finger cutting it off this then became an assassin's trade mark It was not until Leonardo got his hands on the device plans that he was able to realize that it was Being used In an incorrect way due to the way that the gearing and the wire mechanism to extend the blade was working he recognized it was a punch dagger that people were working as a wristblade so instead he reversed the gearing and the wire mechanism so instead of making a fist to pull up the blade you had to extend your hand up work and out as if you were raising your Palm this would project the blade outward but at the same time protect your hands From losing your fingers. The only reason Leonardo was able to understand this is because He looked at it not as an assassin looking at a weapon and how to use it. He looked at this as a tool and how it could be made more efficient and More effective. The functionality and understanding of the blade is clear as day it's just the manufacturing and production as well as the understanding of the gearing system and the trick pulled trigger that allows you to extend your hand upward basically in a grasping motion towards someone's face or neck that extends the blade that is still the problem or was the problem for most assassins until Our assassin boy took it to his best Buddy Leo and said dude can you fix this and Leo was like S*** yeah I can do this in a minute just go Have yourself a sandwich and some coffee I got this
