  • Hyperpigmentation is a catchall term that covers a number of different complaints, but the top three that I see in clinic are melasma, post-acne marks and sun damage - what we call solar lentigines - which are these fixed round brown lesions, flat. They don't change much through the year, but they're a culmination of a lot of UV exposure over time.
    So when it comes to treating hyperpigmentation, while sunscreen is, of course, your first line of attack, I think that in terms of building an active skin care routine, we can break the pigmentation pathway down into steps that helps you understand why a multi-tasking approach with a carefully chosen toolkit of ingredients, will make the biggest difference.

    Thank you Dr. Bunting. My melasma has drastically improved over the last year as I began using sunscreen on my face daily! Also I began to incorporate Azelaic Acid, Retinoids, Vit C & Niacinamide. Bakuchiol is new to me. I’ll have to check that out & your products!     Would love to know more about skincare treatments like microneedling and it’s potential impact on melasma as well as HRT treatments with estrogen. I can’t find any studies available. Thanks for another great video

    Hi Dr. Sam
    Can you please describe how to use these actives in order. Should you use them in night routine or daytime. Also so confused when to incorporate the use of vitamin c serum. I have always thought that vitamin c helps the efficacy of sunscreen to apply in the am before sunscreen, now i see some dermatologist say it can worsen hyperpigmentation if used in the AM and used be used in PM routine instead .

    Excellent! So very helpful. Thanks to your videos I've basically got almost all those ingredients in my routine. What if I use a chemical sunscreen which has UV filters that block UVA and UVB but no iron oxide in the ingredients. Does that mean I'm not protected from visisble light? What is your opinion on The Ordinary's 10% Niacinamide serum?

    I’ve had melasma/hyperpigmentation for a few years and not from pregnancy. It’s taken time finally got a routine down and it’s faded a bit. But now I think I will change my routine now that I’ve seen this video. Thank you, I’m glad I’ve found you. When and how many time a week would I use AHAs

    Can you tell us which ingredients to look out for when dealing with "fungal acne", especially when it comes to moisturiser and SPF.

    Thank you for this video. I've had age spots for years and have found 2% over-the-counter hydroquinone does not do much. I've tried it for years from several reputable brands. I suspect I need prescription strength.

    Hi Dr Sam. Lovely vid as ever but pretty please sort out your sound issues! Firstly, it's too low a level and secondly it sounds a little 'off mic'. I'm a director and have a few free and handy tips for you.... You need to boost your levels if it's a top mic (on the camera) - or better yet, wear a radio mic on your lapel. You can see whether or not it's strong enough a signal if it hits about 2/3 or 3/4 the way up on your monitor... it will be enough without distorting.

    Just in your store and want to purchase your brightly serum as I have hyperpigmentation and it’s melasma (started when I was pregnant 12 years ago and getting worse during the menopause) .     Does this also apply to hyperpigmentation caused by shaving and deodorant in underarms? I tried kojic acid, glycolic acid and niacinamide. Not even hydroquinone did work for me. Now I am giving tranexamic acid a try.

    Hey doc 👋 - I have a stinging / burning reaction to my face wash (LRP hydrating) on my upper lip only! Does this mean my barrier is compromised only on my upper lip ? Currently around my nostril it is very flaky and dry as if it was winter time which is weir ( maybe from the retinol I’m using .5% skin medica ) but the cleanser always burns there even before I used a retinol. Should I stop using the cleanser I love it otherwise!

    I am more confused due to the fact, according of some reading the better for melasma are the natural physical sunscreen, but happens a that some of the sunscreen offers in this Chanel are not 100% physical and have chemical in their Phormula and this is bad for people with melasma and can have backwards reactions, due to inflammation, that cause melasma too, what are your thoughts about not 100%mineral sunscreen ? It is too bad the brands are selling the products as 100% mineral and they still have chemical ingredients, as a consumer looks like we have to learn chemistry when you have issues as melasma for not getting worse

    Dr Sam can retinoids bring up old PIH or melasma hyperpigmentation from beneath the skin ? I use differin for old PIH and on my forehead I came out in crusts which peeled and left me with dark hyperpigmentation spots which after 6 months are still there . I’m scared to use a retinoid now

      Bakuchiol is used as a cure for vitiligo in Indian medicine (Ayurveda). Bakuchiol is also called Babchi in Hindi. I would suggest you look it up on the web and if possible get some advice from an expert. It cured my mom’s vitiligo.
