i have two question>. on parkland method,are we not supposed to add on the daily maitenence dose on top of the fluids calculated by parkland formula... kindly help me explain how use of normal saline causes hyperkalemia
No you don't add. The parkland formula is used for resuscitation fluids. On the other hand the maintenance fluid in 24 hours use the 100/50/20 or the 4/2/1 per hour formula. My opinion
parkland formula is for resuscitation in the first 24 hours to replace the fluids lost adding the maintenance to this cause overload, the maintenance fluid is often started after 24 hours to replace the ongoing insensible losses… The hyperkalemia in patients treated with Normal saline (NS) is due to extracellular shifting of potassium as a result of hyperchloremia associated with the large volume NS infusion.
