Regarding the newer streams: Monster surge only happens on gates that match the current omen symbol. Also, fights happen before the encounter. If you don't beat all the monsters you don't get an encounter. Also again, in a combat encounter, if you roll fewer successes than the sanity/health damage you still take the difference in damage. So if a monster does 3 sanity damage and you only get one success, you still take 2 sanity damage. Same goes for physical damage. If a monster does 4 damage, and you get 2 successes, the monster takes 2 damage, but you also take 2 damage.
