This song is about the pain of self-harm. For some, hurting yourself is a liberating feeling. With this feeling, at some point you no longer have control over your situation and the pain becomes the captain.

Ppl tend to agree that this is a self-reflecting song that describes a person's dark relationship with himself. It could be Till himself since he is well known for his love for pain. The scars on his body are real - he did this to himself for the video of the piano version of "Mein Herz Brennt" which I definitely recommend watching (no live version but official video).

To conquer his stage fright he began to become a persona on stage and isolate himself from his real self. The lyrics also reflect that.
