For the "13 at a table, first to stand will die," that's actually what happened. From Trelawney's perspective, there were 12 people sitting at the table and she was the 13th to sit. However, before Trelawney sat down, Dumbledore stood up. He, out of all the people there, was the first to die. That is because, although it seemed as if there were only 12 at the table, there was a 13th person before Trelawney sat down: Peter Pettigrew. Ron's rat was with Ron, and therefore, 13 people were at the table and Dumbledore was the first to rise.
