    Great video thanks, Ive had MRI indicating I have a fat pad impingement in left knee, and quadricep tendonitis in same knee, its depressing! What i find weird is that I can go on bike as it starts to aggravate after about 20 mins or its under a heavy load, yet I can go for walks, jog on a treadmill and use an Arc Trainer in gym without issues??

    Very informative, thanks. What is the possibility of having both of these at the same time from an overuse injury (cycling)? I have swelling above the patella and have pain with flexion, while also having pain below the patella with full extension. I find that to have the least amount of pain at about 25°. I've had this for over a year now and I've followed several plans from physios without correction unfortunately...

    Excellent explanation and illustration. I now know that what I have is a Hoffa's fat pad situation. Now I just have to figure out what to do about it.

    can standing too long with knee slighlty hyper-extended could causs hoffa fat pad syndrome, also high likely is it to show up in MRI

      @@ClinicalPhysio Took what i learned here to the orthopedic who confirmed that it was a patellafemoral truama injury by looking closer at the MRI. So really do appreciate it. Now we have a plan!

      Great question! I would naturally do it for testing from a diagnostic capacity, but then perhaps you may spend a small period of time offloading extension (but making sure the patient knows that completely stopping extension could have an impact on stiffness)

      @@ClinicalPhysio exactly what I thought m, I have been dealing with this for over 3 months , I hurt me knee bu suddenly stepping out of bed to a deep squat to grab something on the floor and this seems to be my pain; drs say chondromalacia patelle but honestly I think the fat is also a culprit, when I activate my hamsting to do the leg lift as i point my toes to the roof there is a little to hesitation to fully extend my knee, but I don’t want to end un with less ROM compared to my other knee soo I push myself; just wasn’t sure if I am irritating further by forcing it past discomfort; thanks, you’re awesome.

    I have Hoffa’s Fat Pad Syndrome and its actually very painful, the fat pads on both my knees are constantly swollen and always irritated. My physical therapist tells me its because my knees goes slightly inwards which is putting the wrong load on my knee.

    What to do if you've had both injuries and both flexion and extension hurt?

      Hi Ben! I would definitely suggest seeing a physio so you can have this properly assessed

      Same here! Following some isometric exercises with the PT and looking for specialized medical advice, as I also have PF syndrome / Chondromalacia.

    Is it common to have Hoffa’s from overuse with a hip misalignment ?

      Overuse quite possibly… but I’m not personally convinced that the hip alignment plays a consistently significant role in that
