I love how all the medical students were saying how simply you explained the blood clot. The rest of us lay people were thinking "What on earth are you talking about?" Got the big picture, though. Clot goes from leg to heart to lungs. Nothing good comes from that!

This is the first video of yours that I've seen. As I was watching you walk around I kept thinking, "Hm, this hospital looks very familiar. I think I recognize those paintings. I recognize those door signs." Then when you were leaving the hospital I knew exactly where you were. I graduated from the radiology technologist program there last year and walked those same halls for nearly 2 years. At 1:38 I can see my old dorm building peeking over the trees. Such a small world.

This happened to me September 11,2017. I had a femur fracture repair surgeries and the next day I began coughing uncontrollably and my heart rate was at 195 bpm, I was then transferred to the ICU because of a pulmonary embolism. I felt peace during it as if I were going to die or something. Super weird but very thankful my medical team acted fast and saved my life

Thank you for walking us through a CT scan! I’m taking undergrad anatomy and my professor is alway testing us on CTs for lab exams. This allowed me to understand the full purpose and importance of a CT scan, since a picture in a book doesn’t do it justice.

As someone in Nuclear Medicine, its cool to see what PE's look like on a CTPA, rather than our favourite V/Q. Also its really cool to see how much nicer diagnostic CT is compared to the low dose CT's we tack on the end of our studies!

Thank You for taking the time to show this. I had a Bilateral Pulmonary Embolism and wondered what kind of damage that could do to the heart, when it's bilateral? I am still recovering and it has been close to a year, oxygen levels have dipped into the 80's for those wondering. As a middle aged woman, who does not drink nor smoke, I knew something was seriously wrong, when I hardly had enough air in my lungs to speak the words to answer the ER's questions. The pain shot through the front as well as the center of my back, sweating, higher bp and pulse, very nauseated and light headed as well. When you think your dying it's a good idea to get to the ER ASAP.

I don’t trust any radiologist. I always get a second opinion now. Had a radiologist once tell me I didn’t have a kidney stone and my intense pain was probably a pulled muscle. Nope, it was definitely an excruciating kidney store that eventually came out. Ibuprofen didn’t help one bit. I was told on two occasions that I pulled a muscle in my chest. Nope, each time I fractured a rib(lovely bronchitis.) Was told I pulled a muscle in my back. Within a month a my leg went numb, couldn’t walk, take a number 2, and I refused to leave until they helped. Turned out I had 3 herniated discs and one was pushing against my nerve. Again excruciating pain. Was once told my thumb was fine and that I had broken the other hand. Yes, I did break my other hand but what do you know turn out my thumb and the joint on my other hand was fractured.
After that being said... I’d trust this guy. Seems very knowledgeable. Has great experience under his belt. Good work!

My experience with a “pulmonary embolism” last month in our crowded local Canadian ER:
-go in for a post-subluxed shoulder with pain going up my neck and down into my chest (I have EDS so I thought maybe I pinched a nerve when my shoulder went back in).
-triage was like nah, you have a fever too (didn’t feel it)
-bloodwork done while sitting in the “streaming ED waiting room”, doc later calls me into a supply closet (busy ER, nowhere else to discuss in private lol) to tell me I have an elevated d-dimer and asks me to point out where my chest pain is. I tell him it’s coming in from the shoulder. He asks if I have any upper central..and I tell him no, just lower lung (that has been going on for ages)
-get a chest CT, and it shows no PE though half the study was in “expiration” so partially occluded the view, and otherwise just showed small pleural effusions in the lower lungs.
-doc is like, “you’re good” and was sent home..shoulder still killing me. 🙄😂
    🤷🏻‍♀️

my brain cannot comprehend how a thrombus can be in the right atrium and the left ventricle at the same time - how the contractility of the ventricle doesn't somehow affect it/rip it to pieces and how the heart just pumps the long worm through it so that it gets stuck in truncus pulmonalis or arteria pulmonalis dex/sin.

Dr. Cellini: I had a law school classmate (young, healthy guy-28 years old and in shape) of mine who was studying for the bar exam the summer after graduation and out of nowhere he has a pulmonary embolism and DIED! It was so tragic and unexpected. Apparently his wife took him to a clinic a day or two before bc he wasn’t feeling well, but they must have not caught it. Is that a situation where they wouldn’t have thought to do scans to check for an embolism since he was such a young and healthy guy? My heart still aches for his widow and how he had his whole life and career in front of him just to pass away while he was studying for the biggest exam of his life. 😢

Hey, I have a quick question on your thoughts about the future of radiology. I am still an OMS-I and trying to figure out what I enjoy but, are you concerned with the increase in AI for diagnostics and the outsourcing of studies to foreign groups? I know this shift is, in part, the reason IR is becoming so desirable. Thoughts? Thanks! Keep the sweet content rolling

Our radiologists tell us they hate it when there’s an order with no “reason” or “history” with it. They say that it helps tremendously to add the reason and findings that led for the ordering of the particular imaging, as well as some pertinent information regarding the history or family medical history. The radiologists I’ve spoken with all have said they use their own custom method to scan the imaging results and do a full scan no matter what, but with that information they can make sure to pay close attention to the particular areas the coincide with the patient information. Hope that helps.
